နိုက်ထရစ်အောက်ဆိုဒ် Code – အပိုင်း (၁)
သင် မသိဘဲ ပိတ်ဆို့နေခဲ့တဲ့ Pathway
သူမဟာ အားလုံးကို မှန်မှန်ကန်ကန် လုပ်ခဲ့ပါတယ်။
Reflux အတွက် ဆရာဝန်ညွှန်ကြားထားတဲ့အတိုင်း နေ့စဉ် omeprazole သောက်ခဲ့ပါတယ်။ သွားဆရာဝန် အကြံပြုထားတဲ့အတိုင်း မနက်တိုင်း antibacterial mouthwash သုံးခဲ့ပါတယ်။ နေကြာဆီကိုလည်း “နှလုံးကျန်းမာရေးအတွက် ကောင်းတယ်” လို့ ပြောကြလို့ ချက်ပြုတ်ရာမှာ သုံးခဲ့ပါတယ်။ တစ်နေ့ကို ရှစ်နာရီလောက် စားပွဲမှာထိုင်ပြီး အလုပ်ကြိုးစားခဲ့သလို အိပ်ရေးလည်း ကောင်းအောင် ကြိုးစားခဲ့ပါတယ်။
ဒါပေမယ့် သူမရဲ့ သွေးပေါင်ချိန်က တဖြည်းဖြည်း မြင့်လာပါတယ်။ အားအင်တွေ လျော့လာပါတယ်။ အသားအရေက မွဲခြောက်လာပါတယ်။ ခန္ဓာကိုယ်က လေးလံပြီး လည်ပတ်မှုမရှိသလို ခံစားလာရပါတယ်။

2026 ခုနှစ်မှာ အစ္စရေးနိုင်ငံရှိ Weizmann Institute of Science မှ သုတေသီတွေက လူ့ခန္ဓာကိုယ်ထဲက အခြေခံကျတဲ့ signalling pathway တစ်ခုကို ကျွန်ုပ်တို့ နားလည်ပုံ ပြောင်းလဲစေနိုင်တဲ့ တွေ့ရှိချက်တစ်ခုကို ထုတ်ပြန်ခဲ့ပါတယ်။
သူတို့တွေ့ရှိခဲ့တာက လူသိများတဲ့ဆေးတစ်မျိုးဖြစ်တဲ့ sildenafil ဟာ ကင်ဆာဆဲလ်တွေ ပြန့်ပွားဖို့ လိုအပ်တဲ့ cholesterol ကို ရယူအသုံးပြုနိုင်စွမ်းကို ထိခိုက်စေနိုင်တာပါ။
ယန္တရားက တိကျပါတယ်။ ဒီဆေးဟာ cGMP လို့ခေါ်တဲ့ signalling molecule ကို ထိန်းချုပ်ပေးတဲ့ enzyme တစ်မျိုးကို ပိတ်ဆို့ပါတယ်။ cGMP ကတစ်ဆင့် cholesterol ဟာ ဆဲလ်အတွင်း ဘယ်လိုရွေ့လျားသလဲဆိုတာကို ထိန်းချုပ်ပါတယ်။

နေ့စဉ်ဘဝမှာ သာမန်လို့ ထင်ထားတဲ့ ရွေးချယ်မှုတွေကြောင့်လည်း ဖြစ်နိုင်ပါတယ်။
ဆေးဝါးက ပြဿနာဖြစ်တဲ့နေရာမှာ ဝင်ရောက်သက်ရောက်ပါတယ်။

Clinical Explanation – ဆေးဘက်ဆိုင်ရာ ရှင်းလင်းချက်
Nitric oxide pathway ဟာ ဒီလိုအလုပ်လုပ်ပါတယ်။
အဆင့် (၁) – Raw Material
အရွက်စိမ်းဟင်းသီးဟင်းရွက်တွေနဲ့ beetroot ထဲက nitrate တွေကို သွေးထဲ စုပ်ယူပြီး တံတွေးဂလင်းတွေမှာ စုစည်းပါတယ်။
အဆင့် (၂) – ဘက်တီးရီးယားက ပြောင်းလဲပေးခြင်း
လျှာမျက်နှာပြင်ပေါ်မှာရှိတဲ့ သီးခြားဘက်တီးရီးယားတွေက nitrate ကို nitrite အဖြစ် ပြောင်းလဲပေးပါတယ်။
အဆင့် (၃) – အက်ဆစ်နဲ့ နောက်ဆုံးပြောင်းလဲခြင်း
အစာအိမ်ရဲ့ အက်ဆစ်ပတ်ဝန်းကျင်ထဲမှာ nitrite က nitric oxide အဖြစ် ပြောင်းလဲပါတယ်။
Nitric oxide က သွေးကြောတွေကို ပြေလျော့စေပြီး သွေးလည်ပတ်မှုကို ကောင်းမွန်စေကာ ဆဲလ်အတွင်း cholesterol ရွေ့လျားမှုကိုလည်း ထိန်းညှိပေးပါတယ်။

၁။ Proton Pump Inhibitors (PPIs)
Omeprazole, esomeprazole, lansoprazole စတဲ့ဆေးတွေဟာ အစာအိမ်အက်ဆစ်ကို 80–95% အထိ လျှော့ချနိုင်ပါတယ်။
အဲဒီလို pH မြင့်လာတဲ့အခါ nitrite က nitric oxide အဖြစ် ပြောင်းလဲမှုဟာ အများကြီး မဖြစ်တော့ပါဘူး။
PPI တွေဟာ DDAH လို့ခေါ်တဲ့ enzyme ကိုလည်း တားဆီးနိုင်ပါတယ်။ အဲဒီအခါ eNOS ကို တားဆီးတဲ့ ADMA စုပုံလာနိုင်ပါတယ်။

၂။ စက်မှုလုပ်ငန်းသုံး Seed Oils
Sunflower oil, corn oil, soybean oil နဲ့ canola oil တွေမှာ linoleic acid များပါတယ်။
ဒီအဆီတွေ oxidise ဖြစ်တဲ့အခါ သွေးကြောအတွင်းပိုင်းဖြစ်တဲ့ vascular endothelium မှာ oxidative stress ဖြစ်စေနိုင်ပါတယ်။
ဒီဖြစ်စဉ်က eNOS လုပ်ဆောင်ဖို့ လိုအပ်တဲ့ cofactor BH4 ကို oxidise ဖြစ်စေပြီး လျော့နည်းစေနိုင်ပါတယ်။
BH4 မလုံလောက်ရင် eNOS က nitric oxide ထုတ်လုပ်တာ ရပ်သွားပြီး superoxide လို့ခေါ်တဲ့ ထိခိုက်စေနိုင်သော free radical ကို ထုတ်လုပ်လာနိုင်ပါတယ်။
ဆိုလိုတာက သွေးကြောတွေကို ကာကွယ်ဖို့ ဒီဇိုင်းလုပ်ထားတဲ့ enzyme ကပဲ သွေးကြောတွေကို ထိခိုက်စေနိုင်တဲ့ အခြေအနေ ဖြစ်လာနိုင်ပါတယ်။

၃။ Antibacterial Mouthwash
Nitrate က nitrite အဖြစ် ပြောင်းလဲဖို့ လုပ်ဆောင်ပေးတာဟာ ဘက်တီးရီးယားတွေပါ။
Antibacterial mouthwash က အဲဒီဘက်တီးရီးယားတွေကို သတ်ပစ်ပါတယ်။
လေ့လာမှုတွေမှာ chlorhexidine mouthwash ဟာ ပါးစပ်ထဲက nitrate reduction ကို တိုင်းတာနိုင်လောက်အောင် လျှော့ချပြီး သွေးထဲက nitrite ပမာဏကို လျှော့ကာ သွေးပေါင်ချိန်ကို မြင့်တက်စေနိုင်ကြောင်း တွေ့ရှိထားပါတယ်။
ဒီအကျိုးသက်ရောက်မှုဟာ ရက်အနည်းငယ်အတွင်း ပေါ်လာနိုင်ပါတယ်။

၄။ ပါးစပ်ကနေ အသက်ရှူခြင်း (Mouth Breathing)

သွေးဟာ သွေးကြောအတွင်းနံရံတစ်လျှောက် စီးဆင်းတဲ့အခါ ဖြစ်ပေါ်လာတဲ့ ပွတ်တိုက်အားကို shear stress လို့ခေါ်ပါတယ်။
Shear stress ဟာ eNOS expression ကို လှုံ့ဆော်ပေးတဲ့ အားအကောင်းဆုံး physiological signals တွေထဲက တစ်ခုဖြစ်ပါတယ်။
အချိန်ကြာကြာ ထိုင်နေခြင်းက eNOS expression ကို လျော့ကျစေနိုင်ပါတယ်။

eNOS expression ဟာ circadian rhythm နဲ့အညီ ပြောင်းလဲပါတယ်။ မနက်အစောပိုင်းအချိန်တွေမှာ လုပ်ဆောင်မှု အမြင့်ဆုံး ဖြစ်လေ့ရှိပါတယ်။

The Nitric Oxide Code – Part 1: The Pathway You Didn't Know You Were Blocking
She had done everything right. She took her daily omeprazole for reflux, as prescribed. She used antibacterial mouthwash every morning, as her dentist recommended. She cooked with sunflower oil, because it was "heart-healthy." She sat at her desk for eight hours a day, worked hard, and tried to sleep well.
And yet, her blood pressure crept up. Her energy faded. Her skin dulled. Her body felt heavy, stagnant.
She was confused. "I am doing everything I was told. Why is my body turning against me?"
Here is what no one told her.
The Pivot
In 2026, researchers at the Weizmann Institute of Science in Israel published a finding that reframed how we understand a fundamental signalling pathway in the human body. They discovered that a well-known drug, sildenafil, interfered with the ability of cancer cells to access the cholesterol they need in order to spread. The mechanism was specific: it blocked an enzyme that regulates a signalling molecule called cGMP, which in turn controls how cholesterol moves inside cells.
This was a significant discovery. But the pathway the drug illuminated was not a pharmaceutical invention. It is one of the most fundamental signalling systems in human physiology, and it runs continuously in every blood vessel in your body.
The pathway is governed by nitric oxide. It depends on green leafy vegetables, stomach acid, the bacteria on your tongue, whether you breathe through your nose, whether you move, and when you sleep.

The Clinical Explanation
The nitric oxide pathway works like this.
Step 1: The raw material. Nitrates from green leafy vegetables and beetroot are absorbed into your bloodstream and concentrated in your salivary glands.
Step 2: The bacterial conversion. Specific bacteria on the surface of your tongue reduce these nitrates to nitrites.
Step 3: The acidic finish. In the acidic environment of your stomach, nitrite is converted into nitric oxide, which relaxes your blood vessels, improves circulation, and regulates cellular cholesterol traffic.
The six common blockades that disrupt this pathway:
1. Proton pump inhibitors (PPIs). Drugs like omeprazole, esomeprazole, and lansoprazole suppress stomach acid by 80-95%. At that pH, the nitrite-to-nitric-oxide conversion largely does not occur. PPIs also inhibit an enzyme called DDAH, which allows an eNOS inhibitor called ADMA to accumulate. Two separate mechanisms, one drug.
2. Industrial seed oils. Sunflower, corn, soybean, and canola oils are high in linoleic acid. When oxidised, they generate oxidative stress in the vascular endothelium. This oxidises and depletes BH4, a cofactor required for eNOS to function. Without BH4, eNOS stops producing nitric oxide and starts producing superoxide, a damaging free radical. The enzyme designed to protect your blood vessels begins damaging them.
3. Antibacterial mouthwash. The nitrate-to-nitrite conversion is performed entirely by bacteria. Antibacterial mouthwash kills them. Research shows that chlorhexidine mouthwash measurably reduces oral nitrate reduction, lowers circulating nitrite, and increases blood pressure. The effect appears within days.
4. Mouth breathing. Your paranasal sinuses produce nitric oxide continuously. When you breathe through your nose, this nitric oxide is carried into your lungs, where it dilates blood vessels and enters systemic circulation. Mouth breathing bypasses this entirely.
5. Physical stillness. Shear stress, the frictional force of blood moving along the endothelial surface, is the single most potent physiological stimulus for eNOS expression. Prolonged sitting downregulates eNOS expression. The inverse holds equally. Movement is not optional. It is mechanical signalling.
6. Circadian disruption. eNOS expression follows a circadian rhythm, with peak activity in the early morning hours. Late eating, irregular sleep, blue light after dark, and shift work all disrupt this coordination.
You cannot see these blockades. They accumulate quietly, over years, in the same body that is also trying to manage blood pressure, cholesterol, and energy.
