Is a 'wait and see' approach a good idea when a patient presents with 'no scoliosis'?

Children with no signs of skeletal maturity (6-10 years) with a cobb angle of < 10° should be observed at 6-12 month intervals and have Schroth therapy started from the time the curve is detected. When cobb angles are >25 ° , Schroth therapy and brace wearing should be indicated.

Often in clinical practice , we have seen children who are skeletally immature with curves

The goal of Schroth exercises is to de-rotate, elongate and stabilise the spine in a three-dimensional plane.

Here in this video, you see muscular symmetry being restored and a better alignment of the spine with the 3-D corrections. Shoulder traction is used. This is done by reaching outward through the shoulders and elbows during postural correction. This exerts a tension on the ribs and indirectly the spine, to help with spinal straightening and opening the scoliotic concavities.

Breathing is a very important component of Schroth exercises and it involves targeted expansion of the ribcage. This is an active breathing technique, during which we use the diaphragm and intercostal muscles to push the ribcage outward in all directions during the inhalation, and hold the ribcage outward on the exhalation with sufficient tension.

Muscle activation maintains the expansion of scoliotic concavities.
