• Successfully signs a service agreement with OSAN Ability
• Has an approved Home Care Package or relevant aged care funding
• Commences services with OSAN Ability
• The referred client must remain active with OSAN Ability for a minimum of 30 days
• The referrer must have obtained consent from the individual before sharing their details
• The reward will be issued after the 30-day service period is completed
• Only one reward per referred client
• This offer is valid for referrals made during April only
