🖤 A black hole on black ice.

That’s how I’ve come to think about diagnosing occult hernias in women.

Black ice is difficult to see in the first place. Now imagine trying to find a black hole sitting within it.

Occult hernias may have no obvious lump. Symptoms can overlap with gastrointestinal, gynaecological, musculoskeletal and nerve problems. Imaging may not always give a straightforward answer.

And sometimes the biggest clue is simply that the diagnosis you have doesn’t quite explain the clinical picture.

That sentence has taken on a whole new meaning for me as I’ve gone back through my own medical records and tried to understand a presentation from 2019.

The deeper I go into this journey, the more I realise What the NUCK!!! Another Missed Diagnosis. isn't only going to be about my hernias.

It’s going to be about the black ice of diagnosis — and what happens when something important is hiding what doesn't make sense.

🍊 NEW VIDEO: B6 TOXICITY RECOVERY — WHAT IMPROVED & WHAT HASN’T?

More than a year after discovering I had vitamin B6 toxicity, I thought it was time to take an honest look at where I am now.

At my worst, I was dealing with burning and buzzing feet, severe leg cramps, nerve pain and tingling, problems with my arm function, sleep issues, vision changes and a long list of symptoms I didn't understand.

My B6 went from 302 nmol/L to greater than 923 nmol/L — above the laboratory's measurable range.

So what happened after I stopped B6?

In this video I talk about what has substantially improved, what hasn't, how long it took before I started feeling better, and the huge milestone of eventually getting back to running 5 km.

Of course, my body then decided the story needed another chapter. 😂

Enter the hernia detour.

Running is currently on the back burner while I prepare for surgery, and that experience has taught me another important lesson:

Not every symptom I experience now is necessarily B6 toxicity.

I'm also now having B12 injections after testing for deficiencies, with further B12 investigation and another B6 test planned for November.

Perhaps the biggest change has been my attitude towards vitamins.

After B6 toxicity, I wanted nothing to do with them.

Now my approach is much gentler:

Find out what you need — and take what you need.
Not everything.
Not nothing.
What YOU actually need.

It’s easy to focus on what is still not quite right during B6 toxicity recovery.

With everything that has happened with my hernia diagnosis, I sometimes forget to look back and recognise the substantial improvements I’ve made.

🧡 My leg cramps have dramatically improved.
🧡 My arm function has improved.

These were significant symptoms for me, and their improvement matters.

Another thing that has changed is how I approach blood tests. I now regularly check for both deficiencies and toxicities, and I ask whether testing beyond the standard blood panel may be appropriate.

Recovery isn't just about noticing the symptoms that remain.

Sometimes it’s worth looking backwards and recognising just how far you’ve come. 🧡

A firefighter is called to a house because there are signs of fire. They find one plausible source and say, “That must be it,” without checking the rest of the house.

Nobody would consider that a thorough fire inspection.

Yet with persistent pelvic pain, finding a plausible gynaecological, gastrointestinal, musculoskeletal or neurological explanation can sometimes end the investigation.

What if there is another fire?

An occult hernia, by definition, may not produce an obvious lump. You cannot reliably rule out something you never specifically looked for.

Persistent unexplained pelvic or groin pain in women should prompt consideration of targeted assessment for occult abdominal wall and groin hernias and, where clinically appropriate, they should be ruled out rather than simply assumed absent.

People experiencing persistent or recurrent abdominal, pelvic or groin symptoms deserve a proper differential diagnosis. That should include consideration of abdominal-wall and groin hernias when the symptom pattern fits - including occult hernias that cannot necessarily be seen or felt.
