Sydney Health & Care Services is looking for our next Full-Time Service Coordinator to join our Home Care team.
This is a great opportunity for someone with experience in disability, aged care, home care, community services or rostering coordination who enjoys problem-solving, supporting frontline workers and keeping services running smoothly.
As a Service Coordinator, you will play a key role in ensuring our participants receive consistent, responsive and high-quality supports.
You’ll be involved in:
✅ Coordinating roster changes and short-notice call-ins
✅ Arranging shift coverage based on participant needs and worker compatibility
✅ Communicating with participants, families, support workers and internal teams
✅ Supporting incident documentation and escalation
✅ Maintaining participant and service documentation
✅ Assisting with support plans, risk assessments and service agreements
✅ Working closely with our Senior Service Coordinator and management team
This role is suited to someone who can multitask, communicate clearly and make practical decisions in a fast-paced environment.
Office Hours:
Monday to Friday, 9:00am – 5:00pm
There is also a rotational after-hours on-call component shared between Service Coordinators, covering weekday evenings and weekends. Rosters are planned and communicated in advance.

Essential requirements:
✔ Certificate III in Individual Support or equivalent qualification
✔ Minimum 2 years’ experience in disability, aged care, community services or a similar high-demand service environment
✔ NDIS Worker Screening Check
✔ Current First Aid and CPR
✔ Current Driver’s Licence and access to a vehicle
