Osteoarthritis

Knee osteoarthritis is a chronic degenerative joint disease where the cartilage inside the knee gradually breaks down. Cartilage normally acts like a smooth shock absorber between bones. When it wears away, the bones begin to rub against each other, causing pain, stiffness, swelling, and movement problems.

It commonly affects:

√Older adults
√Overweight individuals
√Athletes with previous injuries
√People with poor joint mechanics or muscle weakness

The knee joint mainly involves:

√Femur (thigh bone)
√Tibia (shin bone)
√Patella (kneecap)

& What Happens in OA

In osteoarthritis, several structural changes occur:

1. Cartilage Degeneration
The smooth cartilage becomes:
√Thin
√Rough
√Cracked
Eventually, cartilage may disappear in some areas.

2. Joint Space Narrowing
Since cartilage is thinner, the space between bones decreases on X-ray.

3. Osteophytes (Bone Spurs)
The body forms extra bone around the joint edges to stabilize the knee.

4. Synovial Inflammation
The joint lining becomes irritated and produces inflammatory chemicals causing swelling and pain.

5. Subchondral Bone Changes
The bone under cartilage becomes harder and thicker.

Gluteus minimus muscle is the smallest and deepest of the three gluteal muscles. It sits underneath the Gluteus medius muscle and plays a major role in hip stability, walking mechanics, and pelvic control.



It is located on the outer side of the pelvis, deep to the gluteus medius.

√Origin

Outer surface of the ilium (pelvic bone)

√Insertion

Anterior part of the greater trochanter of the femur

Supply

Superior gluteal nerve (L4, L5, S1)

Supply

Superior gluteal artery

Functions

1. Hip Abduction

√Moves the leg away from the body.

Examples:

√Side leg raises
√Walking sideways
√Stabilizing during standing

2. Internal Rotation of Hip

Turns the thigh inward.

Examples:

√Turning while walking
√Pivoting movements

3. Pelvic Stabilization

√One of its most important functions.
√During walking or single-leg standing, the gluteus minimus helps keep the pelvis level.
Weakness can cause:

√Hip drop
√Poor gait mechanics
√Lower back compensation
√Knee valgus

This is commonly seen in:

√Runners
√People with anterior pelvic tilt
√Individuals with weak glutes or poor core control

Importance

√Weakness Can Cause

Trigger Points/Referred Pain

Causes of Dysfunction

√Prolonged sitting

√Trendelenburg Test
√Checks pelvic stability.

If the pelvis drops on the opposite side while standing on one leg, gluteus minimus/medius weakness may be present.

√Single Leg Balance Test
√Difficulty maintaining balance may indicate weakness.
√Manual Muscle Testing
√Hip abduction resistance testing.
