Endometrial polyp
Typical features (especially on transvaginal USG):
Focal echogenic mass within the endometrial cavity
Well-defined margins, often round or oval
Thickened endometrium (localized rather than diffuse)
May see a feeding vessel on color Doppler → single central vascular pedicle (very characteristic)
Heterogeneous echotexture sometimes due to cystic spaces
Can cause focal bulge of endometrial lining into uterine cavity
Best time to detect:
Early proliferative phase (just after menstruation) → endometrium is thin, so polyp is more visible
Adjunct technique:
Saline infusion sonohysterography (SIS) → clearly outlines the polyp as an intracavitary filling defect
Differential points (important in exams):
vs hyperplasia → diffuse thickening, no focal mass
vs submucosal fibroid → usually hypoechoic, multiple vessels, may cause shadowing
