🚨 MEASLES (RUBEOLA): HIGH-YIELD CLINICAL RECAP 🩺✨
Measles remains one of the most contagious airborne viral infections worldwide, yet it is fully preventable.
📌 Core Snapshot
* Pathogen: Measles virus (Morbillivirus, Paramyxoviridae family).
* Transmission: Respiratory droplets & airborne spread.
* Incubation Period: 7–14 days (Rash usually appears ~14 days post-exposure).
* Infectious Period: 4 days BEFORE to 4 days AFTER the onset of the rash.
🩺 Clinical Presentation
1️⃣ Prodromal Phase (2–4 Days)
The classic hallmark triad—The 3 Cs:
* 🔴 Cough
* 🔴 Coryza
* 🔴 Conjunctivitis
* Associated with: High fever, malaise, and photophobia.
2️⃣ Koplik Spots (Pathognomonic!)
* Small, bluish-white lesions on the buccal mucosa (opposite the lower molars).
* Appears 1–2 days before the rash onset.
3️⃣ The Measles Rash
* Type: Erythematous maculopapular rash.
* Distribution: Starts behind the ears/hairline → Spreads cephalocaudally (downward to trunk and limbs).
* Resolution: Fades in the same order with brownish discoloration and fine desquamation.
🔬 Diagnosis & Workup
* Clinical: High index of suspicion based on history & examination.
* Laboratory:
* Measles IgM Antibody (post-rash onset).
* RT-PCR (Throat/nasopharyngeal swab or urine) for early confirmation.
* CBC: Commonly reveals leukopenia.
⚠️ Key Complications
* Respiratory: Pneumonia (Leading cause of mortality! 🔴), Otitis Media, Croup.
* Gastrointestinal: Diarrhea, Dehydration.
* Neurological: Acute Encephalitis, Febrile Seizures, and SSPE (Subacute Sclerosing Panencephalitis—a rare, late fatal complication).
* Ophthalmic: Keratitis, Corneal Ulceration, Blindness (heightened in Vitamin A deficiency)
💊 Management & WHO Vitamin A Protocol
🟢 Supportive Care
* Airborne isolation precautions.
* Adequate hydration & nutritional support.
* Antipyretics (Paracetamol).
* Antibiotics only if secondary bacterial infection is confirmed.
🟡 Vitamin A Therapy (WHO Guidelines)
Administered once daily for 2 consecutive days:
* 👶 < 6 Months: 50,000 IU
* 👶 6–11 Months: 100,000 IU
* 🧒 ≥ 12 Months: 200,000 IU
(Note: A 3rd dose is recommended after 2–4 weeks if there is clinical evidence of Vitamin A deficiency or severe ocular involvement).
🛡️ Prevention & Prophylaxis
* Vaccination: 2 doses of Measles-containing vaccine (MMR / MR).
* Post-Exposure Prophylaxis (PEP):
* MMR Vaccine: Within 72 hours of exposure.
* HNIG (Immunoglobulin): Within 6 days for high-risk contacts (infants, pregnant women, immunocompromised).
