Catheter-Related Bloodstream Infection (CRBSI) in Hemodialysis Patients: A Practical Clinical Approach

Catheter-related bloodstream infection (CRBSI) remains one of the most serious complications in hemodialysis patients. Early recognition, prompt initiation of antibiotics, and timely catheter management are crucial to reduce morbidity, mortality, and metastatic complications.

1. Initial Evaluation of Suspected CRBSI

Suspect CRBSI in any hemodialysis patient with a central venous catheter (CVC) presenting with:

Fever

Chills or rigors during dialysis

Hypotension

Tachycardia

Altered mental status

Unexplained sepsis

Clinical Assessment

Carefully evaluate for:

Exit-site infection

Tunnel infection

Skin and soft tissue infections

Pneumonia

Urinary tract infection

Intra-abdominal infection

Diabetic foot or other distant sources

Microbiological Workup

Obtain:

Two sets of blood cultures (aerobic and anaerobic)

One set from the catheter hub

One set from the dialysis circuit/peripheral vein
