A certain video landed in my DM’s many times this week. My first reaction was probably not appropriate for this caption. My second? Still not appropriate but it started with WTF 😳

I can’t speak to the full clinical picture but I do have thoughts.

Clinicians often miss PCL tears even when using a highly specific test.

The test isn’t the problem, it’s what happens before you perform the test.

This is part 1 of a 3 part series breaking down the top PCL tests

We’re starting with the Posterior Drawer test and two things you must do before you perform the test: Ensure the hamstrings are relaxed AND the tibia is in a neutral position.

If you know, you know.

The meniscus diagnosis has become the catch-all for acute knee presentations and honestly, a lot of the time it just doesn’t fit the clinical picture, which tells me the clinical reasoning isn’t there to back it up.

MCL injuries get missed (or severity missed), patient expectations get mismanaged, and then everyone wonders why recovery “isn’t going as planned.” Must be the meniscus though, right?

If I sound frustrated, it’s because I am. 😉

Don’t dilute your critical thinking & clinical reasoning by doing too much.

Often it is the cases where you can’t do much of a physical exam that shine a light on the weak spots of your clinical reasoning & assessment process.

✅ Key history questions

These details develop your working diagnosis and drive your physical exam. This is how you move beyond guesswork and start thinking like a specialist.

The best clinicians aren’t doing more tests, they’re asking the right questions and then doing the right tests.
