Our team had a great time at the Seniors Health & Wellness Fair today at PMD Arena in Drayton! 💙

It was wonderful connecting with so many people in our community and having lots of great conversations about staying healthy, active and well.

A big congratulations to the Seniors’ Centre of Excellence on a fantastic event, and thank you to everyone who stopped by to say hello!

A double celebration for Minto-Mapleton FHT! 🧡

We’re incredibly proud to share that Chantelle Koeslag and Jodi Colwill were both nominated for the 2026 NPAO Patient Choice Award! 🎉

Anyone who has had the opportunity to work alongside Chantelle and Jodi knows how much they care about the people they serve. They build strong relationships with their patients, listen, advocate, and bring so much compassion to the care they provide.

Having two of our NPs nominated for this award is something pretty special, and we’re thrilled to see their work recognized.

We know our nurse practitioner Jodi works tirelessly to promote team-based care, both through her work here at MMFHT and through her advocacy work with the Guelph Wellington Ontario Health Team (GW OHT) and the Nurse Practitioner Association of Ontario (NPAO). She is such a strong advocate for primary care and for the role NPs play in our communities, and we’re so proud to see her recognized with this nomination!

This is a permanent, 37.5-hours-per-week position. This is a dynamic role ideal for a nurse who is a self-starter, enjoys taking a leadership role and is comfortable working with digital health tools while maintaining strong patient relationships in a rural primary care setting.

☀️ Heading away on vacation this summer? If you need medical care for an urgent, non-life-threatening concern while you're away, OHIP-funded virtual urgent care services are available for eligible Ontario residents.

Available programs include:
• UrgentCareOntario (adults & children)
• Brightshores Virtual Urgent Care (Grey Bruce region)
• SickKids Virtual Urgent Care (eligible patients under 18)
• CAMH Virtual Urgent Care (eligible mental health & addiction care in Toronto)

These services can help you access care when your regular healthcare provider isn't available.
