🏥 How Quickly Can Home Care Be Arranged After a Hospital Discharge?
Hospital discharges can happen quickly, sometimes with very little time for clients and families to prepare.
Before choosing a Home Care company, ask:
“How quickly can you arrange care when my loved one is discharged from hospital?”
“Will one of your RNs be able to do a thorough onsite assessment to create an in-depth individualised Care Plan prior to discharge?”
“If family is out of town, can you arrange any necessary in-home equipment, ensure the updated medication prescriptions are filled and delivered, shop for fresh food and prepare meals, and provide or arrange the transportation home?”
A senior returning home after surgery, illness, injury, or a prolonged hospital stay, may need extra support with:
✅ Bathing and personal care
✅ Dressing and grooming
✅ Shopping, meal preparation and hydration
✅ Medication reminders
✅ Mobility and transfers
✅ Transportation to follow-up appointments
✅ Housekeeping and laundry
✅ Overnight supervision
✅ Companionship and reassurance
It is also important to ask whether the Home Care company can adjust services quickly if your loved one requires more support than originally anticipated.
A successful transition home requires planning. Whenever possible, contact a Home Care provider well before discharge so an assessment can be completed, Caregivers can be scheduled, and the home can be prepared for the senior’s return. Allowing our RNs the opportunity to visit the client while still in hospital allows them to consult with the hospital RNs, PTs & OTs, and other healthcare professionals involved in the clients care such as Social Workers or Discharge Coordinators, to develop the most accurate Care Plan which drastically increases the likelihood of a successful transition home and recovery.
Before choosing a Home Care company, ask: “If my loved one is discharged tomorrow, how quickly can you have all the appropriate care in place and how will be notified of the arrangements?”
The right support at home can help make the transition from hospital to home safer, smoother, and less stressful for everyone involved.
