Why is high-risk HPV E7 considered an oncoprotein?

One of its best-characterized activities is interaction with the pRB tumor-suppressor pathway. By disrupting pRB–E2F cell-cycle control, E7 can promote continued cell-cycle entry and DNA synthesis.

This simplified mechanism helps explain why E7 expression represents a different biological layer from HPV DNA presence alone. It does not mean that an E7 result, by itself, is a tissue diagnosis or a substitute for established screening and diagnostic pathways.

An HPV-positive result identifies viral presence—but it does not tell the entire natural-history story.

Many HPV infections become undetectable without progressing to clinically significant disease. The risk conversation changes when a high-risk HPV infection persists over time, especially when the same genotype remains detectable.

This is why viral presence, persistence and markers of biological activity should not be treated as interchangeable concepts.

A prospective cohort of 40,399 cervical samples found that CIN3+ risk varied substantially across persistent oncogenic HPV types, with persistent HPV16 carrying the highest observed risk in that study.
