Spannender Fallbericht:
A case report published in Child and Adolescent Psychiatry and Mental Health described a 16-year-old boy who developed severe psychiatric and neurological symptoms alongside vitamin B12 deficiency.
For about a year, he experienced irritability, anxiety, apathy, frequent crying, social withdrawal, poor concentration and memory, insomnia, reduced appetite, and eventually stopped attending school normally. His condition progressed to auditory, visual, and olfactory hallucinations, delusions, thought broadcasting, and impaired reality testing.
He also developed neurological problems including poor balance and coordination, muscle rigidity, slowed movement, and a masked facial expression. His vitamin B12 was only 166 ng/mL (reference range 197–400), and testing suggested B12 malabsorption. An endoscopy showed atrophy of his stomach lining, while a biopsy found H. pylori bacteria colonizing the stomach.
Doctors treated the H. pylori with clarithromycin, amoxicillin, and lansoprazole, while also giving 500 mcg/day of injected vitamin B12 and a low dose of risperidone.
Within just 2 weeks, his B12 had risen to 595 ng/mL, his psychotic symptoms were no longer present, his balance and coordination had normalized, and his depression, anxiety, apathy, crying, abnormal behavior, and movement problems had substantially improved.
Risperidone was then stopped, while B12 injections continued. At 1 month, repeat endoscopy and biopsy confirmed that the H. pylori had been eradicated, and during another 6 months of follow-up, his psychiatric symptoms did not return.
