Patient 36-year-old with abdominal distension and fatigue. Contrast-enhanced axial CT demonstrates a diffusely nodular hepatic contour with heterogeneous parenchymal attenuation, associated splenomegaly and ascites. The combination indicates chronic diffuse hepatic architectural remodeling with portal hypertensive changes. The classic imaging clue is a markedly irregular, nodular liver surface accompanied by

splenic enlargement and free intraperitoneal fluid.

Patient 68-year-old with acute

neurologic symptoms. Noncontrast head CT demonstrates an acute intraparenchymal hemorrhage centered in a lobar cortical-subcortical distribution, with surrounding edema and localized mass effect. The hemorrhage is peripheral rather than deep gray nuclei or brainstem centered. In an older adult, this characteristic hemorrhagic distribution strongly supports the underlying small-vessel process. Classic clue: a lobar cortical-subcortical hemorrhage in an older adult, particularly when recurrent or multifocal.

Patient 16-year-old with progressive headaches and intermittent vomiting. Coronal T2-weighted MRI demonstrates a well-circumscribed intraventricular mass centered near the foramen of Monro, with heterogeneous signal and numerous small internal cystic spaces producing a characteristic multiloculated appearance. Associated ventricular enlargement reflects obstructive hydrocephalus. The lesion's midline intraventricular location and prominent "bubbly" architecture are the classic diagnostic imaging clue.

Patient 12-year-old with recurrent headaches and a recent seizure. Sagittal contrast-enhanced MR angiographic imaging demonstrates a compact nidus of numerous serpiginous, enlarged vascular channels in the cerebral hemisphere, with prominent feeding arteries and draining veins. The vessels show high-flow characteristics without a dominant parenchymal mass or surrounding edema. The classic clue is a compact vascular nidus with enlarged arterial feeders and early venous drainage.

Can you guess the diagnosis? Answer will be pinned after 24 hours.

Patient 10-year-old with

progressive headache, visual impairment, and endocrine dysfunction. Sagittal contrast-enhanced brain MRI demonstrates a predominantly suprasellar, well-circumscribed mixed solid-cystic mass with heterogeneous enhancement and internal low-signal foci, producing mass effect on the optic apparatus and adjacent hypothalamic region. The combination of pediatric age, suprasellar location, mixed cystic-solid architecture, and characteristic internal mineralized foci is highly suggestive of the underlying entity.

Patient 12-year-old with recurrent headaches and occasional seizures since childhood. Axial brain MRI demonstrates a compact intra-axial vascular nidus with numerous serpiginous signal voids and prominent adjacent tortuous vessels, without substantial surrounding edema or mass effect. The clustered vascular channels and enlarged feeding and draining vessels indicate a high-flow vascular shunt lesion. Classic clue: a discrete nidus composed of multiple serpiginous flow voids with enlarged arterial feeders and draining veins.

Patient 12-year-old with seizures and intermittent headaches. Axial FLAIR MRI demonstrates a well-circumscribed intra-axial right temporal lesion with heterogeneous hyperintense signal and internal variable-signal components, producing mild local mass effect without substantial surrounding edema. The lesion is centered in the temporal lobe and has a relatively expansile appearance. Classic clue: a circumscribed temporal intra-axial lesion with heterogeneous FLAIR hyperintensity and limited surrounding edema.

Can you guess the diagnosis? Answer will be pinned after 24 hours.

Patient 52-year-old with vague upper abdominal discomfort undergoes contrast-enhanced CT. Axial portal venous phase imaging demonstrates diffusely decreased hepatic attenuation relative to the spleen, with preserved hepatic contour and relatively hyperattenuating intrahepatic vessels. No focal hepatic lesion, biliary dilatation, or morphologic features of advanced chronic liver disease are evident. The classic clue is diffuse low liver attenuation with conspicuous intrahepatic vessels.

Can you guess the diagnosis? Answer will be pinned after 24 hours.

Patient 62-year-old with chronic right upper quadrant pain and intermittent nausea. Axial CT demonstrates a contracted gallbladder outlined by dense, curvilinear peripheral mural calcification, producing an eggshell-like appearance. The calcification is centered in the gallbladder wall rather than representing discrete intraluminal calculi. This characteristic pattern of circumferential mural calcification is the key diagnostic imaging clue.

Patient 68-year-old presents with sudden-onset left-sided weakness and facial asymmetry. Noncontrast CT head demonstrates unilateral hypoattenuation involving the frontoparietal cerebral parenchyma, with obscuration of the cortical ribbon and reduced gray-white differentiation, accompanied by mild sulcal effacement. No focal hyperdense hemorrhage is evident. The distribution follows a vascular territory and favors an acute ischemic process. The classic clue is early loss of gray-white differentiation with regional sulcal effacement.

Can you guess the diagnosis? Answer will be pinned after 24 hours.
