🔵 Early Management of Acute HF (ESC, 2026)

✅ 1. Clinical assessment

• Symptoms, hx, vital signs
• Assess congestion & hypoperfusion
• Exclude cardiogenic shock & STEMI/STEMI equivalents

✅ 2. Confirm diagnosis

• Measure BNP/NT-proBNP if diagnosis is uncertain
• High → supports AHF
• Low → consider alternative diagnosis
• Do not delay Rx if clinical suspicion is high

✅ 3. Immediate Rx

• Oxygen if SpO₂ 110 mmHg

✅ 4. Reassess after 2 hrs

• Good response: UOP >300 mL or urine Na >70 mmol/L → continue same dose
• Poor response: UOP 900 ng/L; >75 y: >1,800 ng/L)

Rule-out cutoffs:

• BNP
