As we come to the end of our series on Advance Care Planning, I'd like to spend a few minutes focusing on the clinical element and what it means for you.
Throughout this series, we've explored the importance of planning ahead and having conversations about future care. The clinical element brings all of this together by ensuring that your wishes, values, and preferences are considered when decisions about your healthcare need to be made.
From a clinical perspective, you may discuss your current health condition, any treatments you are receiving, and what future care may look like. You can also talk about your preferences regarding hospital admissions, symptom management, where you would prefer to receive care, and any thoughts you may have about treatments or interventions in the future.
These conversations are not about making decisions for you. They are about ensuring you have the information you need to make choices that reflect your values, wishes, and goals. There is no right or wrong answer, and your preferences can change over time.
Advance Care Planning helps ensure that if your health needs change in the future, the healthcare professionals involved in your care have a clear understanding of your wishes and can provide care that is centred around you.
