Could Persistent Postural-Perceptual Dizziness (PPPD) Be Causing Your Ongoing Dizziness?

Most people will experience dizziness at some point in their lives. This might happen after an ear infection, an episode of vertigo, or even when standing up too quickly. Usually, it settles without any lasting effects. But for some people, the feeling of being dizzy or unsteady simply doesn’t go away.

If you’ve been feeling off balance for months or even years, especially if tests have come back normal, Persistent Postural-Perceptual Dizziness (PPPD) could be the reason.

Persistent Postural-Perceptual Dizziness (PPPD) is becoming increasingly recognised by healthcare professionals and is one of the most common causes of long-term dizziness.

What is PPPD?

PPPD is a condition that affects the way the brain processes balance information relating to our body, eyes and inner ear.

It develops after an event that disrupts the balance system, such as:

An episode of vertigo.
An inner ear problem.
A concussion or head injury.
Vestibular migraine.
A significant illness.
A particularly stressful period or panic attacks.
For most people, the brain adapts once the original problem has improved. With PPPD, however, the brain continues to rely on overly cautious balance strategies, making normal movement feel uncomfortable, disorientating or unsettling which can trigger anxiety.

This doesn’t mean there is ongoing damage to the inner ear. Instead, the balance system has become overly sensitive.

What Does PPPD Feel Like?

People describe PPPD in different ways, but some of the most common experiences include:

Feeling unsteady even though they haven’t actually lost their balance.
Feeling overwhelmed in supermarkets, shopping centres or busy streets.
Dizziness when scrolling on a phone, using a computer or watching fast-moving scenes on a screen.
A constant sense of dizziness or light-headedness.
Feeling as though they’re floating, walking on a boat or soft ground.
Symptoms that are worse when standing or walking.
Symptoms tend to be present on most days and often fluctuate. Some days are better than others, but the feeling rarely disappears completely without treatment.

Why Do Busy Places Make It Worse?

Many people with PPPD notice that certain places are particularly challenging. Supermarkets are a common example.

That’s because your brain uses information from your eyes, inner ears and body to keep you balanced. When the brain becomes more dependent on visual information, busy environments filled with movement, bright lights and crowded spaces can become overwhelming.

It’s not unusual for people to start avoiding these situations because they know they’ll feel worse afterwards.

Is It Related to Anxiety?

This is an area that often causes confusion.

PPPD is a genuine medical condition. It isn’t “just anxiety” and the symptoms are not imagined.

Many people with PPPD have already seen several healthcare professionals before receiving the correct diagnosis. While that can be frustrating, getting an explanation for what’s been happening is often an important step towards recovery.

Can It Be Treated?

Yes. Although recovery doesn’t happen overnight and can take several months, many people make excellent progress with the right treatment.

Treatment may include:

Vestibular rehabilitation exercises to help retrain the balance system.
Education about how PPPD develops and why symptoms persist.
Cognitive behavioural therapy (CBT) for people who have developed fear or avoidance around movement.
Medication (prescribed by a doctor) in some cases, where appropriate.
Treatment is tailored to the individual, and many people benefit from a combination of approaches.

When Should You Seek Help?

If you’ve been experiencing dizziness or unsteadiness for several months, particularly if it affects your work, driving, shopping or everyday activities, it’s worth seeking professional advice.

Persistent dizziness shouldn’t simply be accepted as something you have to live with. The right assessment can identify what’s causing your symptoms and help you access the most appropriate treatment.
