Many people do not realise they may be at risk of heart disease until problems develop later on 🫀

Simple lifestyle changes such as improving diet, becoming more active, stopping smoking, reducing alcohol intake and knowing your blood pressure can all help support heart health and reduce the risk of coronary heart disease.

With the help of our trusted partners, we bring together a range of free online health tools to help you better understand your health, including:

✨ Heart age checkers
✨ Blood pressure tools
✨ Diabetes risk assessments
✨ Mood and wellbeing quizzes
✨ Sleep and alcohol calculators

These tools are designed to support healthier choices and greater awareness, but they are not a medical diagnosis.

Trying to compare food labels while shopping can be difficult, especially when sugar, salt and fat levels are not always obvious.

The free Food Scanner app from NHS helps make healthier choices easier by allowing you to scan food and drink barcodes using your phone camera and compare products in seconds. 💙

The app shows simple traffic light ratings, highlights sugar and salt content, and suggests healthier swaps that may support heart health, blood pressure management, weight management and Type 2 diabetes prevention.

Available on the Apple App Store and Google Play Store 📱

Sugary snacks can cause quick energy spikes followed by crashes that leave you feeling hungry again soon after. 🍪

Biscuits, pastries, chocolate bars, and sugary cereals are often higher in added sugars and lower in fibre, which can affect energy levels, weight, and blood sugar over time.

Swapping to fruit, unsalted nuts, natural yoghurt, or oat-based snacks helps keep you fuller for longer while supporting heart and metabolic health. 🍓

Healthier swaps do not need to feel restrictive. Small changes across the week can make a real difference over time.

Are you brushing off ongoing shortness of breath as just getting older? 😮‍💨

Breathlessness that is new, persistent or worsening should be checked. It is not something to ignore.

Shortness of breath can be linked to heart or circulation problems, especially if it happens during everyday activity.

Getting advice early can help identify the cause and reduce risk. 🫀

If you are concerned about symptoms, speak to your GP or pharmacist.

Every five minutes, someone in the UK has a stroke.

For many people, the signs appear suddenly:

👉 facial weakness
👉 difficulty speaking
👉 numbness or weakness in the arm

A stroke is a medical emergency and acting quickly can save lives and reduce long-term disability.

Around 100,000 people experience a stroke each year in the UK, with more than 1.4 million stroke survivors now living with the impact of stroke recovery. 🧠

Knowing the symptoms matters, but so does understanding the wider picture. High blood pressure, smoking, physical inactivity, diabetes, high cholesterol, stress, and poor sleep can all increase stroke risk over time.

Stroke Awareness Month is an opportunity to take your health seriously before a crisis happens.

Small lifestyle changes can help protect your brain and heart health long term. 🫀🌱

Artificial intelligence is already changing healthcare across the NHS, including support for people living with cardiovascular disease and diabetes. 🫀

On the next Home Show, hosted by Friends of Know Diabetes North West London , consultant cardiologist and medical informatics specialist Dr Shelley Rahman Haley will be discussing how AI is being used right now to support faster diagnoses, improve decision-making, reduce pressure on healthcare staff, and help patients access care sooner.

Importantly, this is not about replacing doctors or nurses. It is about using technology responsibly to support healthcare professionals and improve patient care across the NHS.

With more than 2.3 million people living with cardiovascular disease and over six million living with diabetes in the UK, conversations like this matter.

A thoughtful discussion on Friday 15th May around the future of healthcare, digital innovation, and long-term condition support. 📅
