Three pregnancy losses? Two? The number that defines recurrent miscarriage depends entirely on which guideline you are reading.

If you have had more than one pregnancy losses, you may have searched for a straightforward definition of recurrent miscarriage and found three different answers instead.

The UK's RCOG says three or more pregnancy losses. Europe's ESHRE says two. The USA's ASRM also says two.

None of these bodies is wrong. They are answering slightly different clinical questions, and the difference matters because it shapes when investigation should start.

What all three now agree on: pregnancy losses do not need to be consecutive, and for RCOG, they do not need to be with the same partner, to count towards your total.

Whether you have had one pregnancy loss, two, or three, there is a sensible next step for you. In around half of cases, no clear cause is found, and even then, the outlook with supportive care alone is genuinely good.

Today we celebrate one of the most extraordinary milestones in reproductive medicine, the birth of Louise Brown in 1978, the world’s first IVF baby.

Her arrival marked the beginning of a scientific revolution that has transformed millions of lives. Since then, the progress has been remarkable. IVF has moved from experimental to mainstream, with over 21,000 babies born from IVF in the UK in 2024. This amounts to one in every 31 births, which is roughly one child in every classroom. The number of people accessing treatment has tripled over the last 30 years, reflecting both advances in technology and growing accessibility.

Since the HFEA started publishing figures in 1991, UK success rates have climbed steadily: from a 9% live birth rate per embryo transferred for patients under 35 years of age that year, to 38% by 2024. Success rates have improved significantly, and these figures highlight how far we have come.

What has not changed though is how much continuity of care with evidence based practice matters: a consultant-led team that knows a patient's history can build a genuinely personalised plan, rather than starting from a generic protocol each time.

From ICSI treatment to blastocyst culture, fertility preservation, pre-implantation genetic testing, mitochondrial donation, donor gametes/embryos and surrogacy options, to name a few, reproductive medicine continues to evolve at an astonishing pace. What began with one pioneering birth in 1978 is now a global success story built on science, compassion and hope.

Secondary infertility is far more common than people think, and it deserves more conversation.

Secondary infertility means struggling to conceive or carry a pregnancy after you have already had a baby without treatment. In the video, Dr Mugdha Kulkarni explains a few of the reasons this can happen, from age and ovarian reserve to changes left behind by a previous pregnancy or birth.

NHS guidance says to seek help after 12 months of trying if you are under 35, or 6 months if you are 35 or older or have a known risk factor.
