Bit late posting, but better late than never!

We had a fantastic time on the 15th of July at the Family Hub at Leigh opening day!

Our General Practice Assistant team (Jodie,Chelsea and Danine) and First Contact Practitioner team (Matt) had a great time connecting general health into the community; working together to support families in our area!

Thanks again to the Family Hub at Leigh for having us!

The health check is for anyone over the age of 18. The health check will include: -Height-Weight-Calculating a BMI-Blood pressure readings-Measure heart rate

With your consent this will be sent back to your GP if needing further investigations.

We can directly refer and book into any of the PCN services and refer you to the Community Link Workers, Care Co-ordinators , FCP’s, Respiratory Clinic or back to your GP.

Leigh Primary Care Networks first Park Run at Pennington Flash on the 4th of July along with TABA Primary Care Network, Liga Primary Care Network , Wigan GP Alliance, Health First ALW CIC, Be Well Wigan and other local PCNs.
