Therapy is often imagined as “just talking”, which for many people can feel daunting, repetitive, or even unhelpful if they have already spent a long time trying to make sense of what they have been through. But nervous-system informed therapy recognises that trauma, including PTSD, is not held in thoughts alone. It is also held in patterns of survival within the nervous system, the body, and relationships.

This PTSD Awareness Month, we are highlighting that PTSD is not only about memories of the past, but about how the nervous system continues to respond as if danger is still present. This can show up in patterns such as hypervigilance, emotional overwhelm, shutdown, or difficulty feeling safe even in environments that are no longer threatening.

Because PTSD lives in these learned survival responses, healing is not only about gaining insight into what happened. It is about helping the nervous system experience something different in the present. Therapy can support this by gently bringing awareness to unconscious protective patterns and exploring how they are activated in daily life, while also creating repeated experiences of safety, consistency, and attunement.

Over time, these relational and embodied experiences can help the nervous system update its sense of safety, allowing protective responses to soften and flexibility to return.

For many people, the aim is not just to understand their PTSD, but to develop a greater capacity to feel safe in their body, more regulated in moments of activation, and more connected in their relationships.

Recovery from trauma and mental distress often involves many small, supportive strategies that help the nervous system reconnect with feelings of safety and stability. One of these can be creating gentle reminders of regulation throughout the day.

A photograph, a special object, a place in nature, a calming scent, a piece of music, or even a handwritten note can become an anchor for the nervous system. These cues help gently remind the body that safety, steadiness, and connection are possible.

Over time, repeatedly orienting toward these moments can help build greater awareness of what regulation actually feels like in the body, especially during periods of stress or overwhelm.

It is one of the most painful paradoxes of trauma: having an endless well of empathy for the world, but not a single drop for yourself.

As Janina Fisher highlights, many survivors are experts at providing safety and kindness to others while living with a harsh, critical inner voice. If you find it easier to be a caregiver than to be cared for, this could be a survival strategy.

In unpredictable environments, self-criticism and emotional distance often act as a shield. You may have learned to judge yourself before anyone else could, or you decided that staying "less than" was the safest way to exist. But while these patterns protected you then, they create an exhausting "bind" now, making you feel like you don’t deserve the very compassion you give so freely.
