The connection between women's physical health and mental wellbeing remains one of the most underappreciated areas in clinical practice, and some women can carry a greater hormonal vulnerability, particularly those with a history of postnatal depression or premenstrual mood disorders, and they remain the most vulnerable group. My late mother, Lucy, described the triad of post-natal depression, PMS and severe menopause symptoms in the 1970s, 50 years ago.

Premenstrual dysphoric disorder affects up to 5% of women in the UK and can cause severe depression, anxiety, and suicidal ideation, and yet women wait an average of 12 years for a diagnosis, during which many are treated for bipolar disorder or anxiety instead. Menopausal mental health tells a similarly concerning story, with nearly two in three women aged 50 and over struggling with their mental health and almost 90% not seeking help. For women who have previously experienced postnatal depression or severe PMS, the perimenopausal transition can be an especially vulnerable period, and the hormonal thread running through all of these conditions is one that primary care does not always follow.

Hallucinations remain a big problem with AI language models, especially because they are built to keep the conversation going, sound confident, and often agree with the user. That can make them feel reassuring, but it is also why they are not a reliable source of medical advice, where the context of symptoms and the person in front of you need to be taken into deep consideration.
