Support Assessments & Care Planning

Professional Communication & Relationship Building
Working closely with staff, service users, families, and external partners with maturity and confidence.

🎯 WHAT YOU MUST BRING

✔ Significant senior, supervisory, leadership, or management experience in health & social care
✔ Strong background in outreach, supported living, or community‑based services
✔ Proven ability to work independently and take ownership
✔ Excellent judgement, organisation, and problem‑solving skills
✔ Experience overseeing staff and service delivery
✔ A professional, proactive, mature approach
✔ Full UK driving licence + access to own vehicle
✔ Right to work in the UK
