In today’s .ie piece, spoke about what she believe remains one of the biggest challenges in obesity care:

we are still under-diagnosing the disease of obesity.

During her years in emergency medicine, she saw daily the downstream effects of undiagnosed and untreated obesity:

🫀 heart attacks
🫁 pulmonary emboli
🧠 strokes
🩸 type 2 diabetes
📈 and more.

The case for earlier diagnosis and intervention is clear as daylight 🙄

And yet, obesity remains one of the only chronic diseases where patients are still often expected to “prove” they have tried hard enough before they are offered evidence-based treatment or support.

Do you ever think healthcare has become far more comfortable treating the complications of obesity than treating obesity itself? And if so, why? What is the true impact on people’s long term health and quality of life if we don’t do more to treat obesity?

….the impact of treatments like GLP-1’s and GIP/GLP-1’s are not confined to weight loss.

They are reshaping how people live, how they move in the world and thus experience the world. 🌍

That’s why I believe we need to be more precise in our language.

Obesity is a chronic condition (a long-term condition that requires ongoing care), not something that can be “fixed” and then goes away.

Weight loss is often just one part of the picture. Maintaining that change usually requires a structured, long-term approach and ongoing support.

This is because weight itself is a signal of an underlying condition, not the condition on its own.

Understanding this helps explain why there isn’t a quick or simple solution and why long-term care matters.

If you have been on GLP-1 treatment during perimenopause or post-menopause and noticed that results can differ from person to person, recent research helps explain why.

A clinical study published in Scientific Reports found that postmenopausal found that women after menopause may carry more fat around their internal organs (visceral fat), even when overall body fat is similar.

A 2025 review suggests that declining oestrogen is associated with changes in fat distribution, increased inflammation, and reduced insulin sensitivity.

GLP-1 treatments are designed to support satiety signalling. However, the broader metabolic environment during menopause is different.

This means people can respond differently, even with the same treatment.

In a recent episode of the GLP-1 Digest podcast, Prof. Alex Miras and Ashwin Sharma explore a question that often comes up in conversations about weight loss treatment: what is really happening to muscle?

Prof. Miras highlights an important point. While some changes in muscle mass may be observed, this does not necessarily mean a decline in muscle health.

In many cases, people are becoming more active, moving more easily, and improving their physical function (how well the body performs everyday tasks).

In medicine, conditions such as sarcopenia (a condition defined by both low muscle mass and reduced strength or function) are not diagnosed based on muscle mass alone.
