A Story of Expertise and a Second Chance:
We're sharing an incredible case highlighting the critical importance of skilled medical intervention in high-stakes situations.
A patient recently arrived at the dept. Of cardiology, J.L.N. hospital,Ajmer under the care of Drpramod Pareek presenting with an anterior wall mi and severe chest pain.
The patient's condition was particularly concerning, with an ejection fraction of less than 30%.
Angiography revealed a dire situation: a 90% occluded LAD artery and, even more critically, a 90% left main coronary artery occlusion with significant calcification.
During the diagnostic procedure, a significant wedge and pressure drop was observed after catheter introduction, underscoring the extreme fragility of the patient's coronary circulation.
Given the severity, an angioplasty to both the LAD and LMCA arteries was planned. This was a challenging procedure, especially with the risk of further catheter-induced wedge and pressure drops. However, Dr. Pareek's experience shone through.
To mitigate this risk, he brilliantly opted to keep another PTCA wire in the aorta throughout the procedure, a technique that proved immensely helpful in maintaining stability.
After a focused and demanding effort, the team successfully completed the angioplasty with an excellent outcome, navigating all the complexities and challenges.
