POSTERIOR SUPRASCAPULAR NERVE BLOCK
Most clinicians learn the posterior suprascapular nerve block from diagrams. But diagrams don’t help when you’re staring at a live ultrasound and struggling to find the suprascapular notch, the supraspinatus fossa, or the spinoglenoid notch.
In this fast 2–2.5 minute tutorial, Dr. Amod Manocha (Education Without Barriers) demonstrates a stepwise, fool-proof method:
➡️ Start from a clear, undeniable landmark – the medial border of the scapular spine
➡️ Track laterally until you reach the supraspinatus fossa
➡️ Then tilt anteriorly to visualise the suprascapular notch or posterior–lateral for the spinoglenoid notch
This real-scan approach removes guesswork and gives you a reliable roadmap every time.
⭐ WHY TARGET SELECTION MATTERS — AND WHY MOST GUIDES DON’T TELL YOU THIS
Different posterior targets block different components of the suprascapular nerve.
Choosing correctly is the difference between precision and unintentional weakness:
🔹 Suprascapular Notch (More Complete Block)
• Likely to anaesthetise the supraspinatus → possible abduction weakness
• Useful when your aim is wider sensory–motor coverage (perioperative, global analgesia)
🔹 Supraspinatus Fossa (Selective Block)
• May spare anterior supraspinatus fibres → better preservation of abduction
• Ideal for chronic shoulder pain where selective articular fibre blockade is intended
🔹 Spinoglenoid Notch (Highly Selective)
• Primarily targets infraspinatus branches
• Helpful for isolated posterior shoulder pain patterns
This video shows exactly how each target looks on ultrasound and helps clinicians choose the right level based on the clinical goal (pain vs perioperative vs selective articular block).
⭐ WHAT YOU WILL LEARN IN THIS VIDEO
✔ How to reliably identify the posterior scapular spine landmark
✔ Real sono-anatomy of the supraspinatus fossa
✔ How to angle your probe to visualise the suprascapular notch
✔ How to locate the spinoglenoid notch with minor probe adjustments
✔ Understanding target-dependent outcomes (strength preservation vs comprehensive block)
✔ Practical needle approach demonstrated on an actual patient
If your current technique relies on textbook diagrams, this tutorial will reshape how you perform posterior SSN blocks.
