Rapid Maxillary Expansion (RME): Why Does a Gap Form Between the Front Teeth? 🦷
Description
Ever noticed a gap appear between the upper front teeth after wearing a palatal expander? 🤔
This is called a midline diastema, and it is often a normal and expected sign of successful Rapid Maxillary Expansion (RME). As the expander gently widens the upper jaw, the mid-palatal suture opens, creating additional space for proper alignment and improved bite correction.
✨ Benefits of Rapid Maxillary Expansion: ✔️ Corrects a narrow upper jaw ✔️ Creates space for crowded teeth ✔️ Improves bite relationship ✔️ Helps achieve a broader smile ✔️ May improve nasal airflow in selected patients
The temporary gap usually closes naturally during the next phase of orthodontic treatment.
📍Every patient is unique. Treatment should always be planned and monitored by a qualified orthodontist or pediatric dentist.

🦠 Is the Upper Airway Microbiome Linked to Pediatric Obstructive Sleep Apnea?
Emerging evidence suggests that the bacteria living in a child's oral cavity and upper airway may play a role in pediatric obstructive sleep apnea (OSA). But what does the current evidence actually show?
I'm pleased to share our latest publication:
📄 Upper Airway Microbiota in Pediatric Obstructive Sleep Apnea: A Systematic Review
Published in the European Journal of General Dentistry
EJGD_25120992.pdf
🔬 What We Found
✔️ Systematically reviewed 5 observational studies involving 237 children with and without OSA.
EJGD_25120992.pdf
✔️ Several studies reported increased abundance of bacterial genera such as Prevotella, Fusobacterium, Veillonella, Porphyromonas, Haemophilus, and Moraxella in children with OSA.
EJGD_25120992.pdf
✔️ However, microbial findings varied across different upper airway sites, and no single disease-specific microbial profile was consistently identified.
EJGD_25120992.pdf
✔️ The available evidence is limited by small sample sizes, methodological heterogeneity, and moderate risk of bias, highlighting the need for larger, multicenter longitudinal studies.
EJGD_25120992.pdf
💡 Why This Matters
Understanding how the upper airway microbiome changes in pediatric OSA could improve our understanding of disease mechanisms and guide future research into diagnostic biomarkers and targeted therapies.
At present, the evidence is hypothesis-generating rather than confirmatory, and microbiome-based diagnostics or treatments cannot yet be recommended for routine clinical practice.
EJGD_25120992.pdf
Every systematic review helps identify evidence gaps and provides a roadmap for future high-quality research.
Thank you to all my co-authors and collaborators for their contributions to this work.
