This young 11 year old boy visited a consulting clinic with his mother. His mother complained of him breathing mainly through his mouth as well as a previous thumb sucking habit which had stopped quite late and left him with proclined upper front teeth.
A problem list was formulated as follows:-
- Mildly retrusive lower jaw
- End on molar and canine relationship on the left and Class II molar and canine relationship on the right
- Proclined maxillary anteriors
- Maxillary midline diastema
- Non coincident midlines and maxillary midline shifted to left (considering middle of diastema)
- A secondary tongue thrusting habit due to previous thumb sucking habit was observed.
The patient was given a Removable Twin Block appliance with a tongue crib in it to stop the tongue thrusting habit for a year. After 12 months a noticeable correction of the molar and canine relationship was observed bilaterally, but the right side still remained end on.
Tesla passive SL bracket system was bonded and using asymmetric elastics ( Class II on rt and Class III on left) the midlines were corrected and we achieved the final occlusion!✨
Treatment duration was a total of roughly 34 months , just short of 3 years, including the functional appliance phase. The final result is personally one of my favourites and as you can see he was left smiling ear to ear!🔥
P.s.:-The profile improved more so in the frontal view in my opinion, but to be honest the functional phase did not do much 😅 and I got lucky with timing this phase concurrently with his peak growth spurt to use up and redirect it.

Transformation Tuesdays🔥🤩
This young woman was transferred in the initial stages of her treatment from a colleague for continuing her braces journey since she had moved here. Problem list included:-
- Ant open bite tendency (transient)
- Gummy smile
- Patient was not willing for extraction of premolars
- Prominent lower incisor roots(roots closer to labial cortex)
- Class III tendency
The patient agreed for lower third molar extraction and TADs were placed in 36-35,45-46 interradicular regions post 38,48 extraction for lower arch distallization. Later on TADs were also placed in the maxillary anterior region(11-12,21-22 region) for gummy smile correction by intrusion.
Treatment completion took close to 27-28 months as transient sensitivity was felt by the patient in the lower incisors due to slight worsening of the lower root inclination and proximity to the labial cortex which needed to be corrected and the bite was then settled.
Overall the patient was extremely pleased with the outcome and left smiling wide!😃

Wondrous Wednesdays!✨
This female patient aged 29 years old came to us for treatment continuance after having undergone orthodontic treatment elsewhere for close to 20-22 months.
She was concerned with proclined teeth despite the ongoing treatment and wanted that problem addressed but was not willing for extraction of premolars.
Her problem list included:-
- Proclined upper and lower anteriors
- Further deepening of the bite on retraction if no intrusion was planned
- Need for closure of spaces mesial to 16 and 25 without any anchorage loss.
The patient was informed regarding the need for absolute anchorage and some intrusion using miniscrews. Two 1.5x8 mm miniscrews were placed mesial to the upper 1st molars and retraction was planned with a minor intrusive component. Extraction of the upper right wisdom tooth towards the end of treatment was also advised.
Towards the end minor bracket repositioning and intrusion of tooth #11 was done after which incisal edge composite aesthetic restoration was done by Dr . Total treatment duration was around 19-20 months. The final result left her smiling from ear to ear!😃
