Pain isn’t a damage meter — it’s a protective alarm your brain creates, and its volume is shaped by biology, stress, sleep, and environment (the biopsychosocial model). That’s why a “terrible” MRI can come with zero pain, and a perfect scan can come with agony. Hurt ≠ harm.
For us as physiotherapists, that changes everything: pain education, graded movement, and stress management often do more for chronic pain than rest or imaging ever will.
