Hydrodissection in a Small Pupil needs a little more care. Avoid sudden fluid injection and excessive pressure build-up. Inject gently, watch the fluid wave, decompress the bag and allow fluid to escape before proceeding.
Be Gentle, controlled, and Decompress.

What went wrong? A small pupil and a soft cataract. Hydrodissection was done, and suddenly, the iris pr*****ed through the incision. Why did this happen? How could this have been prevented?
