‎مريض في سن ال١٩ يعاني من التهاب حاد في الضاحك العلوي الثاني الأيمن ، ألم مستمر مع كسر في السن وبقاء جزأ بسيط من تاج السن ، كان المريض يعاني وطلب قلع السن فوراً.
‎الإجراءات المتخذة:-
‎سحب العصب وإزالة سبب الألم فوراً، تنظيف القنوات ووضع حشوة الجذر ، تحضير السن وبنائه بإستخدام الألياف المدعمة .
‎الإجراءات الممكن إتخاذها :-
‎تغليف السن وذلك للحفاظ عليه.

Patient Profile & Chief Complaint:A 19-year-old patient presented with severe, acute inflammation (pulpitis) in the maxillary right second premolar. The patient experienced continuous, persistent pain secondary to a crown fracture, with only a minimal portion of the clinical crown remaining. Due to intense suffering, the patient initially requested immediate tooth extraction.Procedures Performed (Interventions):Emergency endodontic access was performed to immediately eliminate the source of pain.Root canal cleaning, shaping, and obturation (root canal filling) were completed.The remaining tooth structure was prepared and reconstructed using a fiber-reinforced filling material .
Recommended Future Management:Full-coverage crown cementation (prosthodontic crown) is indicated to restore function and protect the compromised tooth from future fracture.

‎تم سحب العصب لسن العقل ووضع الحشوة الدائمية.
‎الإجراءات الممكن إتخاذها:-
‎تغليف السن وذلك للحفاظ عليه .
‎في عيادة آسترو القلع هو الحل ما بعد الأخير دائماً.

The patient reported history of sharp, throbbing pain intense enough to disrupt sleep.Clinical & Radiographic Findings:Following clinical and radiographic evaluation, the tooth was diagnosed with irreversible pulpitis (or necrotic pulp), necessitating immediate endodontic intervention (root canal therapy).Procedures Performed:Complete root canal treatment (pulpectomy) was performed on the third molar, followed by the placement of a permanent restoration (filling).Recommended Future Management:Full-coverage crown cementation (prosthodontic crown) is indicated to reinforce the tooth structure and prevent fracture.Clinic Philosophy:At Astro Clinic, extraction is strictly considered the absolute last resort.

‎مريض زار العيادة يشكو من حساسية في أسنانه المغلفة (علماً أن اسنانه جميعها مغلفة بتغليفات الزاركون المدعم بالايماكس) وكان قد وضع التغليفات هذه قبل سنتين ، طريقة تركيب التغليفات كانت بطريقة الجسور (كل اسنان ٢ ملتصقة ببعضها ) -علماً بأن أسنان المريض صحية وكاملة لكن تم برد جزأ كبير منها- مما أدى الى تورم اللثة وتلف التغليفات والأسنان كذلك، بعد الفحص السريري وبالأشعة وجد ت مسافة بين التغليفات والسن وأن طبقة العاج مكشوفة وهي سبب التحسس مع انحسار لثوي .
‎الاجراءات المتخذة :-
‎تم ازالة التغليفات وتبديلها بتغليفات جديدة استغرقت العملية شهراً للوصول الى حالة لثة صحية واسنان صحية تحت التغليفات ، وللوصول الى الشكل المثالي الذي يرغب به المريض.

A patient presented to the clinic with complaints of hypersensitivity associated with previously crowned teeth. Notably, all teeth had been restored with zirconia-based crowns reinforced with lithium disilicate (commonly referred to as zirconia supported by E.max), placed approximately two years prior.

The restorations had been fabricated as splinted units (i.e., bridge-like configuration with every two crowns connected). It is important to note that the patient’s natural dentition was originally intact and healthy; however, significant tooth structure reduction had been performed. This approach resulted in gingival swelling, deterioration of both the prosthetic restorations and the underlying teeth, and compromised periodontal health.

Clinical and radiographic examination revealed marginal discrepancies between the crowns and tooth structure, with exposed dentin surfaces contributing to the patient’s hypersensitivity, in addition to gingival recession.

Treatment performed:

* Removal of the existing splinted zirconia-based restorations
* Replacement with new crowns

The rehabilitation process extended over one month to achieve optimal periodontal health and ensure healthy underlying tooth structure, as well as to meet the patient’s esthetic
expectations.

‎مريض في العشرينات من عمره ، كان قد قام بتركيب حشوات ضوئية تجميلية مباشرة على أسنانه (ما يعرف بالإبتسامة الفورية ، اللؤلؤية، ابتسامة المشاهير بجلسة واحدة) ،
‎بعد تجربة الإبتسامة كان يشكو من رائحة في الفم التهاب لثوي حاد ، انحسار لثوي ، وتثلم في الحشوات ،مما جعل منظرها غير ملائم لإبتسامة المريض.
‎بعد اقناع المريض كانت الإجراءات المتخذة كالتالي:-
‎ازالة الابتسامة
‎تنظيف الاسنان وعلاج اللثة
‎وبعد مرور شهر على الجلسة الاولى كانت اللثة صحية وقد بدأت تعود لحالتها الطبيعية والأسنان صحية ولا يعاني المريض من اي تحسس.
‎الاجراءات التجميلية حسب رغبة المريض:-
‎تبييض الأسنان بالليزر
‎وزيادة طول القواطع الأمامية بإستخدام حشوة بلون السن مطابقة وبنائها بنظام الطبقات لتلائم رغبة المريض وشكل الأسنان.
‎النتيجة:- أسنان صحية، لثة صحية، ابتسامة طبيعية وجميلة للمريض .

Following the procedure, the patient complained of halitosis, severe gingival inflammation, gingival recession, and chipping of the restorations, which resulted in an unaesthetic appearance.

After convincing the patient, the following treatment plan was carried out:

* Removal of the existing composite veneers
* Professional dental cleaning and periodontal therapy

One month after the initial session, the gingiva had returned to a healthy condition, and the teeth were healthy with no reported sensitivity.

‎إزالة التسوس ووضع الحشوة الدائمية بإستخدام العازل المطاطي مع متابعة حالة المريض .

‎الإجراءات المتخذة:-
‎قلع السن وتنظيف منطقة الالتهاب القريبة من العظم مع وصف العلاج الطبي المناسب والتأكيد على المريض بالإلتزام بإجراءات ما بعد القلع .
‎ملاحظة:-
‎الرجاء مراجعة أقرب طبيب عند ،الشعور بألم طفيف، عند تناول الحلويات ، شرب الساخن او البارد، تحسس اسنان، ثلم بسيط ، فراغ، او حتى تجمع الطعام بين الأسنان، وذلك للحفاظ على سلامة أسنانكم وعدم اللجوء للقلع كحل نهائي.✨

* Curettage and debridement of the inflamed area adjacent to the bone
* Prescription of appropriate medications
* Post-extraction instructions were clearly explained, with emphasis on patient compliance

Note:
