⚠️ لأصحاب القلوب القوية
سيدة تعاني من تشمع متقدم في الكبد واستسقاء بطني مقاوم للعلاج، تم تحويلها إلى وحدة الأشعة التداخلية لإجراء تحويلة بابية جهازية داخل الكبد (TIPS).
بعد تعثر الوصول التقليدي عبر الوريد الوداجي بسبب تحديات تشريحية معقدة، تم اللجوء إلى نهج متقدم ونادر عبر الطحال (Transsplenic-Assisted TIPS) للوصول إلى الجهاز البابي وإنجاز الإجراء بنجاح.
استغرقت العملية قرابة خمس ساعات من العمل المتواصل والدقيق تحت التوجيه الشعاعي، وتم خلالها إنشاء التحويلة بنجاح مع استعادة الجريان الدموي المطلوب، دون مضاعفات تذكر.
من الحالات شديدة التعقيد التي تتطلب خبرة متقدمة وصبراً استثنائياً وتعاوناً كاملاً بين أعضاء الفريق.

⚠️ Not for the faint-hearted

A female patient with advanced liver cirrhosis and refractory ascites was referred to our Interventional Radiology Unit for a Transjugular Intrahepatic Portosystemic Shunt (TIPS) procedure.
Due to failure of the conventional transjugular approach as a result of extremely challenging anatomy, an advanced transsplenic-assisted approach was utilized to gain portal venous access and successfully complete the procedure.
The intervention lasted nearly five hours of meticulous work under fluoroscopic guidance, ultimately resulting in successful shunt creation and restoration of adequate portal flow without significant complications.
This case highlights the complexity of advanced portal interventions and the importance of expertise, persistence, and teamwork in achieving successful outcomes.

من أعقد الحالات على الإطلاق في عالم الأشعة التداخلية
سيدة في مقتبل العمر تعاني من متلازمة بود كياري مع تشمع وضمور شديد في الكبد، وكانت تراجع منذ سنوات بسبب تجمع كميات كبيرة من السوائل داخل البطن مع اضطراب كبدي وكلوي متقدم.
تشريحيًا، قد تُعتبر عملية الـ DIPS شبه مستحيلة في مثل هذه الحالات المعقدة، لكن بفضل الله أولًا، ثم بفضل خبرة وحرفية فريق الأشعة التداخلية في مستشفى الملك المؤسس عبد الله الجامعي، تم إنجاز العملية بوقت قياسي وبنجاح مبهر ولله الحمد.
تم استخدام تقنيات شعاعية متقدمة وتقنيات توجيه دقيقة لإتمام الإجراء بأمان تام ودون مضاعفات، مع تحسن سريري واضح على حالة المريضة.

🔹 Case if The Week:
One of the most complex cases in the field of Interventional Radiology
A young female patient suffering from Budd-Chiari syndrome with advanced liver cirrhosis and marked hepatic atrophy had been struggling for years with massive refractory ascites and progressive hepatorenal dysfunction.
From an anatomical perspective, performing a DIPS procedure in such a challenging case could be considered nearly impossible. However, thanks to Allah first, and to the expertise and outstanding skills of the Interventional Radiology team at King Abdullah University Hospital, the procedure was successfully completed in a remarkably efficient time with an excellent outcome.
Advanced imaging guidance and sophisticated interventional techniques were utilized to safely accomplish the procedure without complications, leading to significant clinical improvement in the patient’s condition.
With determination, teamwork, and meticulous planning,
we turn the impossible into reality. ✨

حالة الأسبوع
🔹 مريض خمسيني ،حضر وهو يعاني من نزيف متكرر في الجهاز الهضمي العلوي واستسقاء بطني شديد مقاوم للعلاج نتيجة تشمع الكبد وارتفاع ضغط الوريد البابي.
أظهر التصوير الطبقي المحوري وجود كبد ضامر مع استسقاء شديد ودوالي ضخمة معدية ومريئية.
كما أظهر التنظير الهضمي وجود دوالي كبيرة معدية ومريئية مع علامات نزف حديث واعتلال معدي شديد ناتج عن ارتفاع ضغط الوريد البابي.
تم إجراء عملية TIPS بنجاح عبر الوريد الوداجي الأيمن، مع الوصول إلى الوريد البابي الأيسر بسبب التعقيد التشريحي والتغيرات الضمورية في الكبد.
كما تم إغلاق الدوالي المعدية والمريئية باستخدام عدة coils قابلة للفصل كبيرة الحجم بنجاح تام.
المريض تحمل الإجراء بشكل ممتاز والحمد لله.

🔹Case of the Week: Complex Classic TIPS
A 58-year-old male presented with recurrent upper gastrointestinal bleeding and refractory ascites secondary to liver cirrhosis and severe portal hypertension.
CT scan demonstrated an atrophic liver with massive ascites and huge gastroesophageal varices.
Upper GI endoscopy also revealed large GE varices with stigmata of recent bleeding and severe portal hypertensive gastropathy.
A successful TIPS procedure was performed through the right internal jugular approach with access to the left portal vein due to the complex vascular anatomy and marked atrophic liver changes.
The large gastroesophageal varices were also successfully embolized using multiple large detachable coils.
The patient tolerated the procedure very well, thanks to Allah.

🔹 حالة الأسبوع | Case of the Week 🔹

🔸 مريض شاب حضر بحالة نزيف بولي شديد (Massive Hematuria) بعد إدخال أنبوب تصريف الكلية (Nephrostomy)، مع فقدان يُقدّر بـ 6 وحدات دم.

تم إجراء تصوير شرياني إسعافي للكلى، والذي أظهر وجود ناسور شرياني وريدي (Traumatic Arteriovenous Fistula) مع نزيف نشط في القطب السفلي من الكلية اليمنى، بمحاذاة طرف أنبوب التصريف.

تم التعامل مع الحالة بنجاح عبر إجراء اغلاق فائق الانتقائية (Super Selective Embolization) باستخدام عدة لوالب قابلة للفصل (Detachable Coils)، مما أدى إلى إيقاف النزيف بشكل كامل مع الحفاظ على وظيفة الكلية.

تم الإنجاز بدقة عالية في مستشفى الملك عبدالله المؤسس الجامعي، بالتعاون مع الزملاء:
معتز فرح وباسل خرفان، وبدعم الفريق التمريضي والفني المميز في وحدة الأشعة التداخلية.

🔸 A young patient presented with massive hematuria following nephrostomy tube insertion, with an estimated blood loss of 6 units.

An emergency renal angiogram revealed a traumatic arteriovenous fistula with active bleeding in the lower pole of the right kidney, adjacent to the nephrostomy tube tip.

Super-selective embolization of the fistula using multiple detachable coils was successfully performed, achieving complete hemostasis while preserving renal function.

Procedure performed with precision at King Abdullah University Hospital, in collaboration with my fellows:
Mo3taz Farah and Basel Kherfan, along with the outstanding nursing and technical team in the IR unit.

🩺 فن الإغلاق الوعائي المتقن
سيده راجعتنا تشكو من ألم في الخاصرة اليسرى، حيث أظهر التصوير وجود ورم وعائي عضلي دهني كبير في الكلية اليسرى (Renal Angiomyolipoma) يحتوي على عدة أمّهات دم.
ورم حميد لكنه قد يكون خطيرًا نظرًا لارتفاع خطر النزيف.
تم بحمد الله علاج الحالة بنجاح من خلال فريق الأشعة التداخلية والمسالك البولية في مستشفى الملك المؤسس عبد الله الجامعي.
حيث تم إجراء إغلاق وعائي انتقائي دقيق باستخدام جزيئات PVA للأوعية المغذية، تلاه استخدام اللفائف (Coils)، مما أدى إلى قطع التروية الدموية بالكامل عن الورم مع الحفاظ التام على التروية الطبيعية للكلية.
✨ دقة عالية… نتائج مثالية… وحفاظ كامل على وظيفة الكلية.

The art of Perfect Embolization
A 50-year old lady presented with left flank pain and was found to have a large left renal angiomyolipoma containing multiple aneurysms — a benign yet potentially dangerous tumor with a high risk of bleeding.
The case was successfully managed by the Interventional Radiology and Urology teams at King Abdullah University Hospital.
A super-selective embolization using PVA particles was performed targeting the feeding vessels, followed by coiling, achieving complete devascularization of the tumor while preserving the native renal vasculature.
✨ Precision, expertise, and organ preservation at its finest.

🔥 حالة الشهر : قصة إنقاذ تدفق الدم 🔥

من الحالات النادرة التي تُجسد قوة التكامل بين التخصصات:

مريض ستيني، أجرى سابقاً عملية قلب مفتوح (CABG)، عاد إلينا يشكو من:
⚠️ ألم صدري
⚠️ دوخة وعدم اتزان
⚠️ ضعف شديد في الذراع الأيسر

📌 التشخيص كان دقيقاً وخطيراً:
تضيقات شديدة في الشريان تحت الترقوة الأيسر أدت إلى:
• ضعف تروية القلب عبر LIMA
• متلازمة Subclavian Steal وتأثيرها على الدماغ
• نقص التروية للطرف العلوي الأيسر

💡 الحل لم يكن تقليدياً… بل تدخلاً دقيقاً عبر الرسغ:
✔️ فتح الشريان بالكامل
✔️ تركيب دعامة متطورة (10 mm Stent)

🎯 النتيجة؟
استعادة التروية بشكل متزامن إلى:
❤️ القلب
🧠 الدماغ
💪 الذراع الأيسر

وتحسن سريري ملحوظ أعاد للمريض جودة حياته — ولله الحمد.

A 65-year-old post-CABG patient presented with angina, dizziness, and left arm fatigue.
Diagnosis revealed severe left subclavian artery stenosis causing:
• LIMA compromise, poor flow
• Subclavian steal syndrome
• Poor left arm perfusion

🔹 Successfully treated via left radial approach with stent deployment.
✅ Excellent clinical recovery.

🔥 حالة نادرة… وتحدٍ جديد يُضاف لسجلّ التميّز 🔥
بعد عامٍ كامل على نجاح واحدة من أعقد عمليات التبز (TIPS) لعلاج الاستسقاء المقاوم ودوالي المريء النازفة لمريض في العقد السادس من العمر، عاد المريض إلينا هذه المرة بتحدٍ مختلف وخطير: نوبات متكررة من الاعتلال الدماغي الكبدي (Recurrent Hepatic Encephalopathy)، أثّرت بشكل واضح على وعيه وتركيزه… رغم استنفاد العلاجات التقليدية.
🔹 القرار لم يكن سهلاً… لكنه كان حاسماً.
بالتنسيق الدقيق مع فريق الجهاز الهضمي الزميلين المتميزين :د.قصي طوالبه والدكتور طارق التميمي، تم التوجه نحو حل دقيق يُوصف بـ “السهل الممتنع”: تضييق التبز (TIPS Reduction) باستخدام تقنية Hourglass، مع تقليل قطر الشبكة من 10 ملم إلى 6.5 ملم.
✨ النتيجة؟
تمت العملية بدقه ونجاح مع
تحسّن دراماتيكي خلال ساعات قليلة فقط!
استعاد المريض وعيه الكامل، تركيزه، وتوازنه الذهني… ليغادر المستشفى في اليوم التالي مبتسماً، بكامل إدراكه، وبحالة سريرية مميزة.
🔹 Rare. Complex. Rewarding. 🔹
One year after a very complex TIPS procedure for refractory ascites and recurrent UGIB, the patient returned with recurrent hepatic encephalopathy refractory to medical therapy.
After meticulous multidisciplinary discussion, a TIPS reduction using the hourglass technique was performed, reducing the stent diameter from 10 mm to 6.5 mm.
⚡ The turnaround was remarkable — dramatic clinical improvement within hours, and discharge the next day fully conscious, oriented, and stable.

إنجازٌ جديد يُضاف إلى قسم الأشعة التداخلية في
مستشفى الملك الموسس عبد الله الجامعي:
رجل ستيني يعاني من تشمّع متقدم مع ضمور شديد في الكبد واستسقاء بطني مقاوم للعلاج، اضطره لسحب السوائل مرتين أسبوعياً.
ماراثون طبي استمر 6 ساعات تُوّج بإجراء TIPS معقّده باستخدام تقنية :
القسطرة المزدوجه من الرقبه والكبد.
تم تحويل ناجح لمسار الدم داخل الكبد.
خبرةٌ تُحدث الفرق… ومهارةٌ تصنع الأمل.

A 6-hour medical marathon: A complex TIPS procedure was successfully performed using a *transhepatic–transjugular gun-sight technique* , restoring rapid porosystemic flow and relieving refractory ascites.
