🫁 PLEURAL EFFUSION — Short Notes

Definition:
Pleural effusion is the abnormal accumulation of fluid in the pleural space between the visceral and parietal pleura. The fluid can compress the lung and affect breathing.

Causes:

Pneumonia and other infections

Tuberculosis (TB)

Heart failure

Cancer

Pulmonary embolism

Liver disease/cirrhosis

Kidney disease

Autoimmune diseases

Trauma or surgery

Signs & Symptoms:

Shortness of breath

Chest pain, especially with breathing

Dry cough

Reduced chest expansion

Decreased breath sounds

Dullness on percussion

Types:

1. Transudate – commonly associated with heart failure or cirrhosis.

2. Exudate – commonly caused by infection, cancer or inflammation.

3. Hemorrhagic – contains blood, e.g. trauma or malignancy.

4. Chylous – caused by leakage of lymphatic fluid.

Diagnosis:

Chest X-ray

Chest ultrasound

CT scan

Thoracentesis with fluid analysis

Treatment:

Treat the underlying cause

Diuretics when appropriate, such as in heart failure

Antibiotics when bacterial infection is present

Therapeutic thoracentesis for significant/symptomatic effusion

Chest tube drainage in selected complicated cases

Complications:

Respiratory distress

Empyema/infection

Lung collapse (atelectasis)

Pleural fibrosis

Recurrence
