Treated for prostatitis. Twice. And the tests came back clear both times.
If that is familiar, here is something most men are never told: around nine in ten chronic prostatitis cases involve no bacterial infection at all. Which is why the antibiotics changed nothing.
The pattern we hear in clinic — pain in the perineum, testicles or tip of the p***s, burning during or after passing urine, pain that is worse after sitting, a feeling of not emptying fully, and flare-ups that follow stress rather than infection.
One recognised cause is the pelvic floor: muscles that have been tight and guarding for so long they have stopped letting go. When the tests keep coming back clear, the muscles are worth assessing.

Today is World Physiotherapy Day.
Physiotherapy is not one job. It is one profession with many specialities — musculoskeletal and sports, orthopaedic and post-surgical, neurological, cardio-respiratory, paediatric, and pelvic health and lymphoedema.
Most people have only ever met one of them, which is why so many think physio is for backs, knees and footballers.
Ours is pelvic health and lymphoedema. Kenya has roughly fourteen certified lymphoedema therapists for the whole country, and most people wait years before anyone names what they have.
If it involves muscle or movement, it is worth asking about. You do not need a diagnosis before you come.

Three years ago, this room was empty.

We opened on 3 September 2023. Victoria had already spent two decades in physiotherapy — what Nairobi did not have was a clinic where pelvic health was the whole point, rather than the last item on someone else's longer list.

Then people started walking in. Women six weeks after a birth, and women thirty years after one. Women carrying swelling nobody had named since their cancer treatment ended. Men after prostate surgery, who are told the least of anyone.

Three years on, there are three of us. None of it was built by us — it was built by every person who decided to ask.

Five things that turned out to have names.
August on this page came down to one idea, told five ways: pain that gets dismissed usually has a mechanism behind it, and mechanisms have names.
Lymphoedema — it is not water. It is protein-rich fluid, which is why diuretics do not treat it.
Chronic pelvic pain in men — treated as prostatitis, twice, with clear tests both times. Antibiotics do not loosen a muscle.
Endometriosis — the surgery helped and the pain stayed, because excision removes tissue, not tension.
Tailbone and sitting pain — you have tried every cushion. No position is safe for eight hours.
Vaginismus — your body is not refusing. It is guarding. A reflex, not a decision.
Different conditions, and underneath most of them the same sheet of muscle holding on.
If one of those names is the first you have heard for something you have lived with for years, that is worth acting on rather than living around.

Your body is not refusing. It is guarding.
If a tampon has never gone in. If you’ve rebooked the same smear three times. If s*x hurts at the entrance every single time, and someone once told you to relax and have a glass of wine.
There’s a name for it: vaginismus.
The pelvic floor tightens involuntarily when pe*******on is attempted. It’s a reflex — the body has learned that this hurts, and it protects the area before you’ve decided anything at all. You are not doing it. That’s exactly why you can’t stop doing it.
Which is why being told to relax has never worked. You cannot consciously release a reflex. If relaxing were the answer, you’d have solved this years ago.
What does work is teaching the muscle to let go — graded, paced work with a pelvic health physiotherapist, moving at a speed you set and never faster. It takes time. It is not endured, it is treated.
One more thing. Postponing the appointment isn’t a character flaw. Avoidance is a symptom of the condition, and it’s one of the reasons this goes unnamed for so long.
Our therapists are women. Assessment is a conversation first — nothing is examined that you haven’t agreed to, and you can stop at any point.
