🟦 AMH & AFC — Ovarian Reserve Assessment (2026 Update)

သားဖွားမီးယပ် fertility consultation လာတဲ့ လူနာအများစုက "ကျွန်မရဲ့ egg reserve ကောင်းပါသလား" လို့ မေးလေ့ရှိပါတယ်။ AMH (Anti-Müllerian Hormone) နဲ့ AFC (Antral Follicle Count) ဟာ ovarian reserve ကို အကဲဖြတ်ရာမှာ အဓိက test နှစ်ခုဖြစ်ပေမယ့် ဒီ result တွေကို မှန်ကန်စွာ interpret လုပ်ဖို့ အရေးကြီးပါတယ်။

🔬 Key Clinical Points:

1️⃣ AMH ဆိုတာ ဘာလဲ — Anti-Müllerian Hormone ကို ovarian follicles ရဲ့ granulosa cells တွေက ထုတ်လုပ်ပြီး remaining egg pool ရဲ့ size ကို reflect လုပ်ပါတယ်။ Cycle-day independent blood test ဖြစ်လို့ ရာသီစက်ဝန်း ဘယ်ရက်မဆို စစ်လို့ရပါတယ်။

2️⃣ AFC က transvaginal ultrasound နဲ့ 2-10 mm antral follicles တွေကို early follicular phase (cycle day 2-5) မှာ ရေတွက်တာပါ။ Operator-dependent ဖြစ်ပြီး cost-effective ကောင်းမွန်ပါတယ်။

3️⃣ AMH vs AFC — Research အရ predictive power အတူတူနီးပါးရှိပါတယ် (kappa agreement ~0.8)။ အကောင်းဆုံးက နှစ်ခုလုံးကို combine လုပ်ပြီး clinical decision ချမှတ်ဖို့ပါ။

4️⃣ AMH Interpretation — 3.5–4 ng/mL = High/PCOS feature ဖြစ်နိုင်။

5️⃣ POSEIDON Classification (2016) — Low prognosis လူနာများကို age + AMH/AFC + previous response အပေါ်မူတည်ပြီး Group 1-4 အဖြစ် stratify လုပ်ပါတယ်။ Stimulation protocol နဲ့ counseling အတွက် အသုံးဝင်ပါတယ်။
