-Electrolytes-Normal
-Cervical Spine Xray-Mild OA Changes

🔆🔆SGLT-2inhibitor တွေက eGFR baseline မသိဘဲ ကျောက်ကပ်ကာကွယ်တယ် ဆိုတိုင်း လျှောက်ထည့်လို့ မရနိုင်ပါ🔆🔆

လူနာက အသက် ၇၆ အမျိုးသမီးပါ ဆီးချို Controlled မရလို့ Left Big toe Amputation ပါလုပ်ထားဖူးတဲ့လူနာပါ

Amputationကိုလွန်ခဲ့တဲ့ ၆ နှစ်လောက်က လုပ်ထားတာပါ

လူနာအိမ်က လာပြောတာက လူနာ ဆီးချိုတွေ ၅၀၀ /၆၀၀ ကျော်တက်နေပြီး ဖျားနေတယ် ဆိုပြီး လာခေါ်တာပါ

ပုံမှန်ဆိုအိမ်ပင့် လိုက်လေ့ မရှိပါ( အိမ်မှာဆိုလုပ်ရခက်တဲ့ အရာတွေရှိလို့ပါ) ဒါပေမယ့် အခြေအနေဘာမှန်းမသိလို့ သွားကြည့်တော့ လူနာက GCS-14/15,BP-90/60, PR-110,Sp02 95%,Fever 100 , RBS 560 ,နဲ့ ဆိုတော့ Sepsis Suspectနဲ့NS0.9% 500ml ချိတ်ပြီးဆေးရုံလွှဲလိုက်တာပါ

မလွှဲခင် ဆေးကိုစစ်ကြည့်တော့ SGLT-2 Inhibitor(Dapagliflozin 10mg) ပါနေလို့ withhold ခိုင်းလိုက်ပြီး ဆေးရုံလွှဲလိုက်ပါတယ်

သိုပေမယ့် လူနာက လုံးဝ ဆေးရုံမသွားပါ

ဒီလိုနဲ့ မဖြစ်တာနဲ့ နောက်ရက် ဖုန်းဆက်တော့ မသွားဘူးဆိုတော့ ဒီတိုင်းထားမဖြစ်တာနဲ့ Insulin ပြောင်းထားပြီးAntibiotic(CS1)Statထိုးထားပြီးသွေးဖောက်ကြည့်တော့
အခုလိုပဲ eGFR-5.5, Cr-600 ကျော် , increased Anion Gap Acidosis နဲ့ တွေ့လိုက်ရပါတယ်

မေးလိုက်တော့ ပြနေကျ ဆေးခန်းက လူနာအတွက် Baseline Investigation တောင် မလုပ်ထားဘဲDapa 10 ထည့်ထားတာပါပဲ

လူနာကလည်းဆေးခန်း မသွားနိုင်တော့ ဒီတိုင်းပဲ ဆေးသွားပြီး ဆေးခန်းကပဲ ဝယ်ဝယ်သောက်နေတာပါ

အခုတော့ AKI ဝင်နေပါတယ်

အခုက Dapa ကြောင့်AKI ဖြစ်တယ်လို့ တတ်အပ်ခေါင်းစဉ်တပ်နေတာ မဟုတ်ပါ

အခုလူနာဆို AKI ဖြစ်နိုင်နေတဲ့ အကြောင်းအရာများပါတယ်

ဒါပေမယ့် ဘလိုပဲဖြစ်ဖြစ် Sepsis ကြောင့်ပဲ AKI ရရ/ Volume Depletion ကြောင့် AKI ပဲရရ Dapaကို ဆက်တိုက်သောက်နေတာက Precipitate လုပ်နေတာပါပဲ

ကျောက်ကပ် ကာတယ် ဘာကာတယ် ညာကာတယ် ဆိုပြီး Online ပေါ်က ကြားပြီး SGLT-2 Inhibitorကိုယ့်သဘောနဲ့ ကိုယ် ဝယ်သောက်နေတဲ့လူနာတွေ တောင်ရှိကြပါတယ်

Note- ဆရာကြီးလုပ်တာ မဟုတ်ရပါ/ Dapa , Empa မထည့်ခင် renal function လေး Assess လုပ်ဖို့ လိုအပ်တာလေး ပြောချင်လို့ပါ

42-year-old lady with progressive vision loss on right side of the eye for 1 year
No Headache
No Ptosis
No Squint
No Opthalmoplegia
No Nystagmus
Pupil Light Reflex-
Left side- Normal on both direct and consensual reflex
Right side- Normal on consensual reflex but pupil alittle bit dilate when the torch light swing to the right side(RAPD+)
Visual Field - Normal on left side
patient can see nothing while examining her right side( Right visual field defect)
Fundoscope- Optic Atrophy on right side

No underlying medical condition

HbA1C - normal
BP-110/70mmhg

Then refered to the neurologist and ophthalmologist all investigations were done except CT/MRI
No AACG ( Opthalmologist)
ANA-1:80(trace)
ENA- Negative
B-12level- Normal
BCR-NR
VDRL- NR
Anti-aquaporin-4- Negative
Anti MOG- Negative
pANCA- weakly Positive
cANCA-Negative
CP(A)-Normal
Ur,EL,Cr- Normal Range
Lipid- Normal

23-year-old female presenting with progressive non-scarring patchy hair loss for about 2 weeks.

Last 5 months, this lady was admitted to hospital due to paracetamol overdose and fortunately she recovered completely.

70-year-old lady presenting difficulty in walking of 1 week duration

Lower Limb examination
Tone-Normal both limbs
Power-4/5 both limbs
Reflex-Normal , EPR-equivocal
Coordination- Normal

Sensory- Normal on both limbs and no sensory level(STT)
PCT- normal

Distal muscle- No wasting and can do planterflex/dorsiflexion of both ankles.

Upper Limb- No pronator drift/ Normal hand grift

Head/Face/Throat- No specific features

Lumbosacral spine Xray- Mild lumbar spondylosis and no compression features

And she walked along and there was Gait abnormality
