A complex case.

Mr R, aged 65, presents with ostial insufficiency of the left internal saphenous vein, with a trunk diameter of 8 mm.
He is treated with 3% foamed chemical ablation (Lauromacrogol).
We meet him 6 months later with a large superficial thrombosis 5 cm from the femoral crook.

We propose 45 days of fondaparinux 2.5 (Calisto study) followed by 1940 Pioon laser ablation.

The internal saphenous vein is severely compromised, full of synechiae.

The 400-micrometre radial PIOON MEDICAL LASER probe will allow us to advance over nearly three-quarters of the length, passing through the synechiae.

We will then apply 35 J/cm and complete the remainder with 2% Lauromacrogol foam.
The procedure will be comfortable and brief, allowing for a rapid return to normal activities.
