𝗟𝗮𝘀𝘁 𝘆𝗲𝗮𝗿, 𝗜 𝘀𝗽𝗼𝗸𝗲 𝘄𝗶𝘁𝗵 𝗮 𝗺𝗶𝗱𝘄𝗶𝗳𝗲 𝗳𝗿𝗼𝗺 𝗮 𝗿𝘂𝗿𝗮𝗹 𝗔𝗳𝗿𝗶𝗰𝗮𝗻 𝗰𝗼𝗺𝗺𝘂𝗻𝗶𝘁𝘆.

She told me about a night she still cannot forget.

There was no doctor.
No ambulance.
No stable electricity.
No digital support system.
No enough staff.

Just one exhausted midwife…
trying to save two lives at once.

A mother was bleeding.

The baby was struggling.

And somewhere between fear, silence, darkness, and delay…

both lives were lost.

What broke her was not only the loss.

It was the feeling that she had been left alone inside a system already overwhelmed by workforce shortages, maternal mortality, and global inequality.

And while the world is racing into an AI-powered and digitally-driven healthcare future…

many midwives are still fighting battles that should never exist in 2026.

That conversation stayed with me.

Because the future of healthcare must not only be digital.

It must be human.

It must be inclusive.

And it must not leave behind the very people who bring life into the world.

As the world marked International Day of the Midwife on May 5th under the global theme:
“One Million More Midwives”

I believe the world does not only need more midwives.

Because no midwife should save lives while the world forgets theirs.

And no midwife should be left behind in the future of healthcare.

A tiny bite shouldn’t have the power to stop a life. 🦟🚫

​Malaria is a master of disguise, but it’s a "criminal" we can catch. Whether it’s recognising the first signs of a fever, sleeping under treated nets, or supporting global initiatives to eliminate the threat entirely, every action counts.

Most people with MCAS spend years being told nothing is wrong.

Normal labs, normal tests, and a body that reacts to almost everything.

If that sounds familiar, keep reading.

MCAS stands for Mast Cell Activation Syndrome.

It means your mast cells (the immune cells responsible for triggering reactions) are firing off constantly and unpredictably.

Heat, cold, stress, exercise, certain foods, chemicals, even your own hormones can set them off.

The reactions show up differently in every person, which is exactly why it gets missed for so long.

Here’s what to do if you recognise these signs:
–Start keeping a reaction diary this week.
–Every time you have a symptom, note the time, what you ate, where you were, and what you were exposed to.
–Patterns will start to emerge within 2 weeks and that information is gold when you bring it to a practitioner who understands MCAS.

Acute Scrotal Conditions (Sudden, Often Painful)
1. Testicular Torsion:
Cause: Spermatic cord twists, cutting off blood supply (bell clapper deformity).
Signs: Sudden severe pain, high/horizontal testicle, absent cremasteric reflex. Surgical emergency.

2. Epididymitis:
Cause: Infection (STIs 35/hetero), urine reflux.
Signs: Gradual pain/swelling, tender epididymis, present cremasteric reflex, possible fever/dysuria.

3. Orchitis:
Cause: Viral (Mumps) or bacterial (secondary to epididymitis).
Signs: Testicular inflammation, pain/swelling, systemic symptoms (fever, malaise).

4. Appendix Te**is Torsion:
Cause: Twisting of small vestigial appendage.
Signs: Less severe pain, upper pole localization, sometimes “blue dot sign,” intact cremasteric reflex.

5. Testicular Rupture:
Cause: Severe blunt trauma.
Signs: Extreme pain, massive swelling/ecchymosis, disrupted testicle on ultrasound. Surgical emergency.

6. Acute Hydrocele/Hematocele:
Cause: Rapid fluid (hydrocele) or blood (hematocele) accumulation (trauma, inflammation).
Signs: Rapid scrotal swelling, discomfort.

Chronic Scrotal Conditions (Gradual, Often Painless/Mild)
1. Varicocele:
Cause: Dilated pampiniform plexus veins (usually left-sided).
Signs: “Bag of worms” feeling, worse standing, may affect fertility.

2. Spermatocele (Epididymal Cyst):
Cause: Benign sperm-containing cyst in epididymis.
Signs: Painless, smooth lump (superior/posterior to testicle), transilluminates.

3. Hydrocele (Chronic):
Cause: Fluid in tunica vaginalis (idiopathic, inflammation).
Signs: Painless fluid-filled swelling, transilluminates.

4. Testicular Mass (Tumor):
Cause: Testicular cancer (seminoma, non-seminoma).
Signs: Painless, firm lump, no transillumination. Urgent urology referral.

5. Chronic Epididymitis/Orchitis:
Cause: Unresolved acute infection or inflammation.
Signs: Persistent discomfort

As a health worker, one thing I see very often is this:

A patient walks in and immediately says, “Nurse, I know what is wrong with me. It’s malaria.”

But when we start asking questions about symptoms, past illnesses, medications, allergies, or recent exposures… the information is either incomplete or left out entirely.

Your health history is very important in helping us make the right diagnosis and give the right treatment. Sometimes what feels like malaria may actually be something completely different.

When you visit a health facility, please help us help you:
• Share your symptoms clearly
• Mention any medications you have taken
• Tell us about past illnesses or allergies
• Answer questions honestly and completely

Self-diagnosis can sometimes delay proper treatment.
