NOTICE TO MY PATIENTS and MEDICAL ASSISTANCE PROVIDERS

In light of the increasing reports of fake and falsified medical certificates being used to request medical assistance or medical leaves, I am implementing additional security and verification measures for all medical certificates issued under my name effective today, July 4, 2026.

These measures specifically the QR codes and validity portals are intended to protect my patients from misuse of their personal and medical information; prevent the unauthorized reproduction, alteration, or falsification of medical certificates; and to allow concerned institutions to verify the authenticity of certificates when necessary.

Please be advised that medical certificates issued prior to the implementation date of these additional security measures, bearing my signature in blue ink, remain valid and authentic.

Thank you for helping us uphold honesty, accountability, and trust in the use of medical documents.

This was our patient that came for a check up last Tuesday. She was advised for admission. She was recently discharged in order for her to attend her final defense.

Her mother messaged our page yesterday to ask how to avail the free HCQS. I I told her na makadto lang since wala man kami ga ask requirements and since patient namon we have her Electronic Medical Record (EMR) na ga use siya HCQS.

Her mom came almost closing na with prescription upon discharged. As I went through her medications I could already calculate how much they would need to spend in a month to sustain it. It’s crushing me. I know the feeling (since both of my parents were also in maintenance and it’s not something just for Hypertension and Diabetes, it’s more).

I gave her about a month of free supply of HCQS and she was thankful for it. Then later on found out that her daughter was not able to take her medications for a long time, hence, the need of admission.

Ginhambalan ko man si maam na mag seek help sa PCSO or sa DSWD since ang help na mahatag whatever the amount is “awat pa na.”

Lord, ginapray gid namon na i-bless mo kami, gaan mo kami suppliers na makahelp samon and sa patients namon sustain sang ila maintenance na indi man sila ma ayawan pangita and mabalda financially.
