- PWD ID or ang valid ID
- Immunization record para sa mga pasyente nga 5 years old or below
- PhilHealth ID / MDR
- Parent / Guardian’s Valid ID

Today’s HPN and DM Club meeting was all about keeping our community healthy. The HRH of Department of Health, PCF Staffs, BHW and BNS conducted cardiovascular disease (CVD) risk assessments and gave out free maintenance medicines to our members.

A big thank you to our local health workers and to everyone who attended.
