By every public health measure, she did everything right. Whether she survives after that depends on something she has no way of checking beforehand: is the facility actually ready?

Some of the Philippines' highest maternal death tolls come from regions with decent roads and nearby hospitals, proof that distance isn't the deciding factor. Readiness is.

MNDRS and MNDSR. Just one letter apart, but they serve two very different purposes.

One records that a maternal or neonatal death occurred. The other examines why it happened and identifies the actions needed to prevent similar deaths in the future.

A health facility may have an excellent reporting system but no effective review process. When that happens, deaths are accurately counted, yet the system failures behind them remain unaddressed.
