Details of incident: The patient arrived at HEMS, with lacerated wound at the head. Patient was accidentally hit during playing of basketball. Mr. Ederson Salvacion HEMS on duty medically assist the patient. vital signs taken and recorded.

Injuries/NOC : Vaginal Bleeding ( 13 weeks and 1 day pregnant)

Action taken: vital signs taken and referred to the nearest hospital

Details of incident:
RHU HEMS on duty receive a call thru radio from MDRRMO, reported that their is a pregnant woman experiencing vaginal bleeding at Sta.Fe. Upon gathering details, HEMS immediately proceeded to the location. Upon arrival, patient was conscious with vaginal bleeding. Initial assessment was done, vital sign taken by Darren Yosures Lying-in on duty and Jillian Perez Hems on duty. The patient was transported to Donsol District Hospital and endorsed to the nurse on duty for further evaluation and management.

Injuries/NOC : Body Weakness, infected wound (bed sore) at Lumbar area

Action taken: The patient was handled carefully into the ambulance, placed on a lateral position to prevent pressure on the spine area. Vitals signs was taken and recorded, transported to Donsol District hospital for further medical evaluation and management.

Details of incident: Patient arrived at RHU HEMS accompanied by childrens,obviously weak and suffering from severe pain, initial assessment was done by staff on duty Mr. Ken Cantonjos and immediately referred to the nearest hospital facility.

Referred to Donsol District Hospital for further evaluation and management

Details of incident:Reported thru call by Ms.Jillian Perez, patient complaining abdominal pain accompanied by vomiting(more than 3x) and body weakness in their residence. Vital sign taken and recorded by Marciano Losamia and Lorie ibañez Hems on duty. Coordinated and referred to BRHMC for further evaluation and management.

Action taken: The patient needs medical assistance or an ambulance to be taken to the hospital.

Details of incident: The mdrrmo called rhu hems for the patient who needs medical assistance because he can't walk because his left knee hurts, the patient has been feeling it since yesterday and the patient was taken to the donsol district hospital so that he can be given first aid because of knee pain.
Conducted/Medically assisted by Mr. Ederson Salvacion HEMS on duty, vital signs taken and recorded, and transpoted at donsol district hospital for further evaluation and management.
