The Sagay City Primary Care Facility warmly invites all Sagaynons with hypertension and/or diabetes to become members of the newly launched Sagay City Hypertension and Diabetes Club.
Under the theme “Prevent, Protect, Prosper: A Unified Fight Against Hypertension and Diabetes,” this online platform streamlines your registration so you can take control of your health, prevent complications, and connect with a supportive community.

A united, empowered, and health-conscious Sagay City with significantly reduced cases and complications of hypertension and diabetes through accessible healthcare, healthy living, and community collaboration.
Our Mission
The Hypertension and Diabetes Club of Sagay City shall serve as a community-based support organization committed to promoting awareness, early detection, proper treatment, and healthy lifestyle practices for individuals with hypertension and diabetes.
Through patient empowerment, active community participation, and strong collaboration among healthcare providers, barangay officials, and local stakeholders, the club aims to improve the quality of life of every member and build a healthier Sagay City community.
Who Can Join?
All residents of Sagay City who have hypertension, diabetes, or both are highly encouraged to register.
