The American Academy of Pediatrics (AAP) has reaffirmed that mother’s own milk is the optimal nutrition for very low birth weight infants in the NICU.

When mother’s own milk is not available, insufficient, or contraindicated, the AAP recommends pasteurized donor human milk to help reduce the risk of serious complications, including necrotizing enterocolitis (NEC). 🛡️

Donor human milk plays an important role in protecting vulnerable infants and supporting improved outcomes when maternal milk is not available.

CAN HIV+ MOMS BREASTFEED?

For many years, the message surrounding HIV and breastfeeding was straightforward: avoid breastfeeding entirely. Today, evolving evidence is allowing for a more nuanced and compassionate conversation.

Recent guidance from the American College of Obstetricians and Gynecologists acknowledges that when a parent is consistently taking antiretroviral therapy and maintains a sustained undetectable viral load, shared decision-making should be part of the discussion.

While the risk of HIV transmission through human milk becomes very low under these circumstances, it is not completely zero. This highlights the importance of individualized, evidence-based counseling that respects both medical safety and parental autonomy.

Healthcare is gradually moving away from blanket restrictions and toward informed, collaborative care centered on the family’s values, circumstances, and goals.
