A young male presented with complaints of right iliac fossa pain for the last 5 days. He had been given injectable a**lgesics and sent home multiple times. On abdominal examination, there was marked muscle guarding in the RIF region, while classical tenderness was difficult to elicit due to the patient’s obesity. After the relevant investigations, the patient was planned for laparoscopic appendectomy. Intraoperatively, a subserosal catarrhal appendix was found. It was a very challenging appendectomy, but the procedure went smoothly. The patient is doing well and has been discharged home. Post-operative follow-up will continue.

The management of diabetic foot ulcers requires offloading the wound, daily saline or similar dressings to provide a moist wound environment, débridement when necessary, antibiotic therapy with or without surgical intervention.
