🔹A challenging case of carcinoma head of pancreas causing a distal C*D stricture with upstream biliary dilatation.
▪️Cholangiogram demonstrated a tight distal C*D stricture with proximal dilatation of the C*D and intrahepatic biliary channels.
▪️Successful biliary decompression was achieved with placement of a 10 × 80 mm partially covered self-expanding metallic stent across the stricture.

👏A challenging case of post-cholecystectomy cystic duct leakage succesfully perfomed by efforts of gastroentrology team, anesthesia and surgery dept of Allied -1 hospital.

▪️43 yr old female , history of lap cholecystectomy 21 days presented with abdominal pain, distension , reduced urine output. Being managed on line of peritonitis and AKI
▪️USG: collection of 4.5x3 cm in gallbladder fossa.
▪️MRCP: leakage from gallbladder stump, perhepatic collection 16x6 cm .
Pigtail catheter placed

🔹ERCP performed by Prof Hafiz Dr Mughees Ather : showing , normal C*D, cholangiogram showing cystic duct leak. Straight plastic biliary stent 10Fr x 10 cm successfully placed across leakage

A challenging case of a 13-year-old boy following blunt abdominal trauma, initially managed conservatively on the Surgical Floor, presented with significant dilatation of both the intrahepatic and extrahepatic biliary systems.
🔹Further evaluation suggested an intraluminal thrombus within the biliary system, creating a complex obstruction requiring coordinated multidisciplinary management. IR guided embolization done .
🔹The ERCP was performed by Sir Mughees Ather (HOD GASTRO) under general anesthesia in the ERCP Suite, AHF-1. Cholangiography demonstrated marked biliary dilatation, and successful bilateral biliary stenting was performed:
Right system: 7 Fr × 12 cm stent & Left system: 10 Fr × 10 cm stent

Case of 36 yr Old male , biopsy proven case of keratinizing squamous cell carcinoma, moderately differentiated, at lower esophagus.
EGD showed growth at 30 cm extending to 39 cm.
