One infection that deserves particular attention is influenza, commonly called the flu.

For someone with healthy lungs, influenza may cause fever, cough, body aches and tiredness. But in a person with chronic lung disease, the same infection can cause much more significant breathing problems.

When a doctor tells someone, “You have COPD,” one of the first questions is often:

“But I have never smoked. How can I have COPD?”

It is a reasonable question.

Smoking is the most important individual risk factor for COPD in many populations, but smoking is not the only way COPD can develop.

The 2026 GOLD report recognises that COPD can result from exposure to harmful particles and gases other than to***co smoke, as well as factors affecting lung development and ageing. GOLD notes that non-to***co risk factors account for a substantial proportion of COPD worldwide.

Therefore, being a lifelong non-smoker does not rule out COPD.

However, if you have never smoked, your diagnosis deserves careful evaluation. It is important to confirm that you actually have persistent airflow obstruction and to look for the factors that may have contributed to your lung disease.
