ERCP for Distal Cholangiocarcinoma

Partially Covered Metallic Stent (pcSEMS) Placement Successfully Performed

A patient presenting with obstructive jaundice and features of gastric outlet obstruction underwent advanced therapeutic ERCP.
🔹 Compression was noted at the duodenal bulb.
🔹 Adult scope negotiated with difficulty after careful manoeuvring.
🔹 Selective C*D cannulation achieved using guidewire and sphincterotome.
🔹 Cholangiogram revealed a malignant stricture extending from the mid C*D up to the ampulla.
🔹 Endoscopic sphincterotomy performed successfully.
🔹 A partially covered self-expanding metallic stent (pcSEMS 100mm × 10mm) was deployed across the stricture into the right IHBD.
🔹 Free flow of purulent bile achieved with effective biliary drainage.
Patient has also been advised duodenal stenting for management of associated gastric outlet obstruction.
Timely endoscopic management of malignant biliary obstruction helps relieve jaundice, control cholangitis, and improve quality of life.
