Aerobic Exercise During Pregnancy: Benefits and Safety

Aerobic exercise can be an important part of a healthy pregnancy. For women without medical or obstetric contraindications, regular moderate-intensity activity can support cardiovascular fitness, physical function, and maternal well-being.

What Is Aerobic Exercise?
Aerobic exercise involves rhythmic movement of large muscle groups that increases heart rate and breathing. During pregnancy, suitable examples include brisk walking, swimming, and stationary cycling.

Why Is It Important During Pregnancy?

Regular aerobic activity may help:
Improve cardiovascular fitness and endurance.
Reduce the risk of gestational diabetes and excessive gestational weight gain.
Support healthy physical function and overall well-being.
Prepare the body for the physical demands of labour and daily activities.

Evidence also suggests that prenatal exercise programmes may reduce the likelihood of cesarean delivery. However, aerobic exercise cannot guarantee vaginal birth or prevent a medically necessary C-section.

Which Aerobic Exercises May Be Suitable?

Depending on individual health and fitness, suitable options may include:

Walking: A practical, low-impact option for many pregnant women.
Swimming: Provides cardiovascular exercise with support from the water.
Stationary cycling: Allows aerobic exercise with less risk of falling than outdoor cycling.
Pregnancy-adapted aerobic activities: Exercises modified to accommodate comfort, balance, and changing physical capacity.

How Can Physiotherapy Help?

A physiotherapist can assess physical capacity, movement tolerance, pregnancy-related symptoms, and previous exercise experience to help develop an individualized exercise programme.

For most pregnant women without contraindications, guidelines recommend gradually working toward at least 150 minutes of moderate-intensity aerobic activity per week. Women who are new to exercise can begin with shorter sessions and progress gradually.

At moderate intensity, you should generally be able to talk but not sing comfortably.

When Should You Seek Professional Assessment?

Consult your obstetric healthcare provider before starting an exercise programme if you have pregnancy complications or concerns about exercise safety.

Stop exercising and seek medical advice if you experience vaginal bleeding, fluid leakage, chest pain, dizziness, or painful regular contractions.

Pelvic Floor Muscle Training During Pregnancy: Supporting Pelvic Health

During pregnancy, the pelvic floor muscles support the pelvic organs and help maintain bladder and bowel control. Pregnancy and childbirth can place additional demands on these muscles, making pelvic floor training an important part of prenatal care.

Types of Pelvic Floor Training
Different training approaches target different aspects of pelvic floor function:

Pelvic Floor Muscle Contractions (Kegel Exercises): Improve muscle strength and support bladder control.

Pelvic Floor Muscle Relaxation: Develop the ability to release unnecessary muscle tension.

Pelvic Floor Coordination Training: Improves coordination between muscle contraction and relaxation.

Pelvic Floor Endurance Training: Focuses on maintaining appropriate muscle activity over time.

Each approach has a different purpose, and the most suitable training depends on individual needs.

What Does Research Show?
Research suggests that pelvic floor muscle training during pregnancy can reduce the risk of urinary incontinence during pregnancy and after childbirth, particularly when training is appropriately prescribed and performed consistently.

Supporting Pelvic Health During Pregnancy:
Effective pelvic floor training involves more than simply strengthening the muscles. Strength, relaxation, coordination, and endurance all contribute to healthy pelvic floor function.

Changes in Sexual Health During Pregnancy

Pregnancy can bring many physical, hormonal, emotional, and pelvic-floor changes that may affect a woman’s sexual comfort and function.

Some women may experience:

• Reduced sexual desire
• Vaginal dryness or discomfort
• Pain or discomfort during in*******se
• Difficulty relaxing the pelvic-floor muscles
• Changes in body comfort or confidence

How Can Physiotherapy Help?

A women’s health physiotherapist can assess pelvic-floor muscle function and identify whether weakness, excessive muscle tension, poor relaxation, or other musculoskeletal factors may be contributing to symptoms.

Depending on the individual, physiotherapy may include:

• Pelvic-floor assessment and education
• Relaxation and breathing strategies
• Appropriate pelvic-floor exercises
• Advice on comfortable movement and positions
• Individualized rehabilitation when pelvic-floor dysfunction contributes to discomfort

Research suggests pelvic-floor interventions may have benefits for sexual function, but evidence specifically during pregnancy is still developing. Therefore, treatment should be individualized rather than assuming that every woman needs pelvic-floor strengthening.

When Should You Seek Professional Assessment?

If pelvic discomfort, pain, or other symptoms are affecting your comfort, relationships, sleep, movement, or quality of life, a professional assessment can help identify possible contributing factors.

Women’s Health Physio Clinic provides women-focused physiotherapy assessment and individualized rehabilitation for pregnancy and pelvic-health concerns.

Physical Activity & Exercise During Pregnancy

Pregnancy does not mean you have to stop moving.

For most women with an uncomplicated pregnancy, appropriate physical activity and exercise are considered safe and can support physical fitness, functional capacity and overall well-being.

What Are the Benefits?
Regular, appropriately prescribed exercise during pregnancy may:

• Help maintain physical fitness
• Reduce excessive gestational weight gain
• Lower the risk of gestational diabetes
• Reduce the risk of hypertensive disorders in some women
• Support strength, mobility and daily function
• Reduce the likelihood of cesarean birth in some studies

A 2026 systematic review and meta-analysis of 88 randomized controlled trials reported beneficial associations between prenatal exercise and several maternal outcomes, although the authors noted concerns about risk of bias in some studies.

What Exercise Can Be Considered?
Depending on the woman's health, fitness level and stage of pregnancy, suitable activities may include:

Walking
Strength training
Stationary cycling
Mobility and stretching
Water-based exercise
Moderate-intensity aerobic activity

Exercise should be individualized and progressed gradually, particularly for women who were previously inactive.

When Should Exercise Be Modified or Stopped?

Women with medical or obstetric complications should receive appropriate assessment before starting or continuing an exercise programme.

Stop exercise and seek medical advice if you experience vaginal bleeding, leakage of amniotic fluid, regular painful contractions, chest pain, dizziness/fainting, severe headache, significant shortness of breath, or calf pain/swelling.

How Can Physiotherapy Help?

A physiotherapist can assess your symptoms, physical capacity, movement limitations and stage of pregnancy and help develop an individualized programme.

This may include exercise education, strengthening, mobility, posture and movement advice, activity modification and safe progression.

The goal is not the same exercise programme for every woman—it is the right programme for the individual.

Varicose Veins During Pregnancy

During pregnancy, increased blood volume, hormonal changes, and pressure from the growing uterus can make it harder for blood to return efficiently from the legs to the heart. This may contribute to varicose veins—enlarged, twisted veins that commonly appear in the legs.

What are varicose veins?

Varicose veins are enlarged and twisted superficial veins, usually seen in the legs when blood does not return efficiently toward the heart.

Why can they occur during pregnancy?

Pregnancy can contribute through:

• Increased blood volume
• Hormonal changes that relax blood vessel walls
• Pressure from the growing uterus on pelvic veins
• Reduced venous return from the legs
• Prolonged sitting or standing

Common symptoms

Women may experience:

• Visible enlarged or twisted veins
• Leg aching or heaviness
• Swelling around the ankles or legs
• Itching or pressure around the veins
• Symptoms that worsen later in the day

How can physiotherapy help?

Physiotherapy does not remove varicose veins, but it can help manage symptoms and maintain safe movement during pregnancy.

Depending on individual needs, physiotherapy may include:

✅ Calf-muscle pump and ankle-movement exercises
✅ Safe walking and pregnancy-appropriate physical activity
✅ Advice on positioning and movement
✅ Education about prolonged sitting or standing
✅ Individualized exercise guidance according to pregnancy stage and symptoms

Compression garments may also be recommended when clinically appropriate.

⚠️ Important:Sudden swelling of one leg, especially with pain, warmth, redness, or tenderness, requires prompt medical assessment, as these symptoms may indicate a blood clot rather than ordinary varicose veins.

Constipation During Pregnancy: How Can Physiotherapy Help?

Constipation is a common problem during pregnancy. Hormonal changes, reduced physical activity, and pressure from the growing uterus can make bowel movements difficult.

What Is Constipation?

Constipation means having difficulty passing stools, passing hard stools, or experiencing fewer bowel movements than usual.

Why Does It Occur During Pregnancy?

Increased progesterone slows bowel movement.
The growing uterus may put pressure on the bowel.
Reduced physical activity can contribute to constipation.
Iron supplements may make constipation worse.
Low fiber and inadequate fluid intake can contribute.

Common Symptoms

Hard or dry stools
Difficulty passing stools
Excessive straining
Abdominal discomfort and bloating
Feeling that the bowel has not emptied completely

How Can Physiotherapy Help?

Physiotherapy can support constipation management through:

Safe exercise: Walking and pregnancy-appropriate exercises to support bowel function.
Breathing techniques: Diaphragmatic breathing to promote relaxation.
Pelvic-floor relaxation: Improving muscle coordination during bowel movements.
Toilet positioning: Education to reduce unnecessary straining.

Adequate water, dietary fiber, and medical advice are also important.

When Should You Seek Help?

If constipation is persistent, painful, or affecting your daily activities, seek professional assessment. Severe abdominal pain, vomiting, or blood in the stool requires medical attention.

Pregnancy-Related Musculoskeletal Rib Pain

Pregnancy can bring many changes to posture, breathing mechanics, and the movement of the rib cage. As the uterus grows, the rib cage may gradually expand and the muscles around the chest and upper back may experience increased stress.

For some women, these changes can contribute to musculoskeletal rib pain, particularly during the later stages of pregnancy.

What is it?

Pregnancy-related musculoskeletal rib pain refers to pain or discomfort around the ribs and surrounding muscles that is related to mechanical and musculoskeletal changes during pregnancy.

Why can it occur during pregnancy?

Several factors may contribute, including:

• Expansion and altered movement of the rib cage
• Changes in posture as pregnancy progresses
• Increased load on the thoracic and upper-back muscles
• Changes in breathing mechanics
• Prolonged sitting or certain positions
• Reduced mobility or muscle tension around the chest and upper back

Common symptoms

A woman may experience:

• Pain or tenderness around the ribs
• Discomfort with prolonged sitting or certain positions
• Upper-back or side-of-chest discomfort
• Pain that becomes worse with particular movements
• Difficulty finding a comfortable sleeping position
• Tightness around the chest or upper back

How can physiotherapy help?

A physiotherapist can assess the musculoskeletal factors contributing to the symptoms and develop an individualized management plan.

Depending on the assessment, treatment may include:

• Postural and movement education
• Gentle thoracic and rib-cage mobility exercises
• Breathing and relaxation strategies
• Appropriate stretching and strengthening exercises
• Activity and position modification
• Advice for comfortable daily movement and sleep positions.

The aim is to improve comfort, mobility and function while keeping exercise and treatment appropriate for pregnancy.

When should you seek professional assessment?

If rib or chest-area pain is severe, persistent, worsening, or associated with shortness of breath, chest pain, fever, dizziness, severe abdominal/right-upper abdominal pain, headache or visual changes, medical assessment is important because not all rib-area pain during pregnancy is musculoskeletal.

If the pain is affecting your sleep, movement, exercise, work, or daily activities, a professional physiotherapy assessment can help identify contributing factors and guide an individualized rehabilitation plan.

Pelvic Floor Dysfunction During Pregnancy

Pregnancy brings many changes to a woman’s body, and the pelvic floor is one of the areas that works harder throughout pregnancy.

As the uterus grows and body weight and pressure change, some women may develop pelvic-floor symptoms even before childbirth.

What is it?

Pelvic floor dysfunction occurs when the pelvic-floor muscles are not functioning properly — they may be weak, poorly coordinated, overly tense, or unable to relax when needed.

Why can it occur during pregnancy?

During pregnancy:

• The growing uterus increases pressure on the pelvic floor
• Hormonal changes affect muscles and connective tissues
• Increased body weight places additional demand on pelvic-floor support
• Changes in posture and movement can alter how the pelvic floor works

Pregnancy itself is considered an important risk factor for pelvic-floor dysfunction.

Common symptoms

A woman may experience:

• Difficulty controlling urine or bowel movements
• Pelvic heaviness or pressure
• Difficulty contracting or relaxing the pelvic-floor muscles
• Pelvic discomfort
• Reduced confidence during physical activity
• Difficulty coordinating breathing and pelvic-floor muscles

How can physiotherapy help?

A women’s health physiotherapist can assess pelvic-floor function and determine whether the muscles need strengthening, relaxation, coordination training, or a combination of approaches.

Depending on the individual, physiotherapy may include:

• Pelvic-floor muscle training
• Breathing and pelvic-floor coordination
• Education about correct muscle activation and relaxation
• Safe pregnancy exercise
• Movement and activity modification
• Functional training for daily activities

Research supports pelvic-floor muscle training during pregnancy, particularly for improving pelvic-floor strength and reducing the risk of urinary incontinence.

Pelvic-floor exercises are not simply “Kegels.” Correct assessment and technique are important because some women may need better relaxation and coordination rather than simply strengthening the muscles.

When should you seek professional assessment?

If pelvic symptoms are affecting your comfort, exercise, sleep, physical activity, or daily life, a professional assessment can help identify what your pelvic floor actually needs.

Gestational Diabetes Mellitus During Pregnancy

Gestational Diabetes Mellitus (GDM) is a type of diabetes that develops during pregnancy. It can affect both maternal and fetal health, but appropriate medical care, nutrition, and physical activity can help with blood-glucose management.

What is it?

Gestational Diabetes Mellitus (GDM) occurs when blood glucose levels become higher than recommended during pregnancy.

Hormonal changes during pregnancy can increase insulin resistance, making it harder for the body to maintain normal blood-glucose levels.

Why does it matter?

GDM may increase the risk of pregnancy complications, so regular monitoring and appropriate management are important.

Commonly, women may have no obvious symptoms, which is why routine pregnancy screening is important.

How can physiotherapy help?

Exercise is an evidence-based part of GDM management when medically appropriate.

A physiotherapist can help with:

• Individualized exercise planning
• Walking and other suitable aerobic activities
• Safe resistance exercises
• Activity modification according to pregnancy changes
• Education about safe exercise intensity and progression
• Monitoring exercise tolerance and functional limitations

Research and clinical guidelines support regular moderate-intensity physical activity, including aerobic and resistance exercise, for women with diabetes during pregnancy.

However, exercise should be individualized according to the woman's medical and obstetric status and used alongside the medical management recommended by her healthcare team.

When should you seek professional assessment?

If you have GDM and are unsure:

• Which exercises are safe for you
• How intensely you should exercise
• How to modify activity as pregnancy progresses
• How to exercise safely with other pregnancy-related limitations

A professional assessment can help develop an appropriate and individualized exercise plan.

Women’s Health Physio Clinic

If you are pregnant with GDM, physiotherapy can support your overall management through safe, individualized exercise and physical-activity guidance.

Restless Legs Syndrome During Pregnancy

Do you experience an uncomfortable urge to move your legs, especially when resting or trying to sleep during pregnancy?

It may be Restless Legs Syndrome (RLS),also known as Willis-Ekbom disease.

What is it?

RLS is a condition that causes an uncomfortable sensation in the legs along with a strong urge to move them. Symptoms typically become worse during rest and at night and may improve temporarily with movement.

RLS is relatively common during pregnancy, affecting approximately 1 in 5 pregnant women in many studies.

Common symptoms

• Urge to move the legs
• Crawling, tingling, pulling, or uncomfortable sensations
• Symptoms worsening while sitting or lying down
• Temporary relief after walking or moving
• Difficulty falling asleep or disturbed sleep
• Tiredness during the day due to poor sleep

Why can it occur during pregnancy?

Pregnancy brings hormonal and physiological changes that may contribute to RLS. Low iron stores or iron deficiency can also be an important contributing factor in some women.

How can physiotherapy help?

Physiotherapy may provide supportive management through:

• Gentle pregnancy-appropriate exercise
• Lower-limb stretching and mobility
• Relaxation strategies
• Activity and rest education
• Sleep-support strategies
• Individual assessment of movement and activity patterns

Because iron deficiency can contribute to RLS, medical assessment may also be important. Do not start iron supplements without professional advice.

When should you seek help?

If symptoms are regularly affecting your sleep, movement, exercise, daily activities, or quality of life, professional assessment can help identify contributing factors and guide appropriate management.
